Towards the end of "My First Day", Dr. Kelso tells J.D. that if he doesn't do his rounds, he'll get the autopsy, plus he'll scratch his name off the list. We then cut to a fantasy shot, where Kelso is angry, with fire in his eyes, as shown in the picture, all while looking at J.D. with a Death Glare.
In one episode, Dr. Cox breaks a pencil. He then destroys a computer. And test tubes. And he even throws a stool to a window! Now see that in 3-D.
