The psuedo-reapers/bloodsuckers didn't exist in Dead Island so it's safe to say yes.
And techland, while they worked on dead island, doesn't own the IP, despite what someone else mentioned in this article. the next DI game is being made by someone else. This is less like DI and more like Far Cry Zombie Edition than anything else.

It defo isn't a real world setting, and I cannot pin point the cities architecture and ethnic/racial make up; and that of the worlds around, to give an idea of what it's based off of.
The closest I'm getting is some kind of...South African (White/accented) Middle Eastern (Arab/accented) thing.
