Some anime, but if you ask me, it's really more suited for "Not What It Looks Like" since it seems to be depicting somebody walking in on two kids in an awkward position, rather than somebody walking in while another is changing.
I have a thought. I recently added an entry under Live Action from The Benny Hill Show where a cross-eyed guy reads the "Ladies" room sign and because of his off vision reads it as "Laddies". I'd need to find a picture, but perhaps a picture of the blurry doubled D with a caption like, "The guy reading this sign is British and cross-eyed. This is not going to end well." Since there are no people in the picture, there's no NSFW material and the joke, while it takes a little maturity to understand, becomes quick to get once you do. Any thoughts?
I'm surprised the Evangelion examples skip the infamous incident with Shinji and Rei in episode 5.
Is there some reason it's considered ineligible, like Rei's lack of reaction to it?

The description is a little confusing. I thought this was where a character was accidentally placed in a situation that makes them look perverted when they weren't, but the description makes it sound like it is some kind of excuse for awkward types or something.

Does the trope "accidental pervert" cover the situation where lovers wrestle and one ends up, for reasons "beyond their control", on top of the other? For example I'm thinking of Star Wars episode 2 where Anakin and Padme are in a field on the planet of Naboo. They end up rolling around on top of each other.

I think there should be some mention as to the self-inserty-ness of this trope. It seems to me like most anime uses it because it allows the writer to maintain the protagonist's characterisation as a moral, pure, nice guy while simultaneously allowing them to grope female characters for the sake of "hilarity".
As a highly-regarded e-psychologist, I would venture as far as to say that this trope is a result of Japan's sexual repression. Hence its more or less exclusivity to anime.
It is a fantasy of boob gropage in a land where wanting to grope boobs makes you a terrible, lecherous person.
What is the source of the picture?
