• Demonic Spiders: The Ubersoldats. They can kill you in an instant second if you're in a room without any cover and they have a huge threshold of health, and even when trying to get headshots on them to kill them quickly, you still have a mere second to get one shot in before going back to hiding. This is even more egregious when the game starts using them a lot more in Episode 6 and certain levels in Spear of Destiny.
  • The flammenwerfers are just as painful in the rear as the ubersoldats; above-average health, able to kill you in an instant second because of how flamethrowers work in Brutal Wolf, and can explode on the player if they accidentally gib them.
• Nintendo Hard: Even more so than the original. The game will absolutely take advantage of the newer and more powerful enemies to get the drop on you, and due to the new engine, certain tactics once viable in the original will get you killed here, like bottlenecking.
• Remade and Improved: Brutal Wolfenstein is considered to be superior to the original game in every single aspect; improved level design, an automap, more weapons, new soundtrack, new enemies, improved boss battles, tons of blood and gore, and also includes Spear of Destiny and several more bonus levels on top of everything else.
• That One Boss:
  • The Death Knight is able to kill a fully equipped player in record time via combining his dual gatling guns and rocket launchers if he so much as looks at them in his direction, while being able to fly around and making himself a harder target to hit, and the arena you face him in is close quarters only while giving the Death Knight some space for himself to fly.
  • The NME comes after the Ubermutant, and it is as nasty as ever. Gatling guns, rocket launchers and flamethrowers, while being as fast as the player all comes together to make a boss that is a nightmare to deal with!
  • Out of the Grosse siblings, Trans is the most hardest because of several factors; you do not have the Shiny Gold Luger, which was made Hans and Gretel easy to deal with compared to Trans, two, the cover provided by Trans' arena is very narrow, forcing you to dodge by crouching and sliding during his hail-fire of lead, and three, when Trans is down to low enough health, several Ubersoldats will come to his aid, making the fight against him even more daunting.
