Unazuki Furuhata, a young waitress who dreamed of finding her first kiss, ends up on the verge of death after her Pure Heart is stolen by a Daimon. So now the Inner Guardians must race to retrieve the heart and keep it safe from both the Daimon and the Outer Guardians.
Tropes in this episode include:
- Almost Kiss: Usagi and Mamoru almost share a kiss in the moonlight but she pauses and asks what their First Kiss was like.
- Bathos: Professor Tomoe has a Laughing Mad fit after confessing that it's been years since he kissed anyone.
- Biblical Motifs: Michiru tells Usagi and Unazuki that Adam and Eve shared what many believed was the world's first kiss.
- Blow You Away: Osouji can create bursts of wind to attack the Guardians.
- Call-Back: Mamoru once again gives Unazuki a ride on his motorcycle while Usagi watches. Usagi even admits that she almost had a heart attack after seeing the two together.
- The Cavalry Arrives Late: Sailors Venus, Jupiter and Mercury show up just after the battle was wrapped up.
- Change the Uncomfortable Subject: In the episode's end, Mamoru tries to distract Usagi from their First Kiss conversation by pointing to the moon and then to an imaginary monster.

- The theme of the day is first kisses with Unazuki longing to gain her perfect one with her boyfriend.
- Discussed between Usagi and Mamoru. Despite having kissed multiple times in the past, neither can remember what their exact first one was like.
- The Four Loves: Haruka references how there are different types of kisses with their own meanings. A kiss on the hand means respect (Agape), while a kiss on the forehead represents friendship (Philia). The odd one out is a kiss on the palm which signifies a need for a favor.
- Freeze-Frame Bonus: Worn out statues of Tuxedo Mask and Sailor Moon can be seen on the pile of rusted toys that Tuxedo Mask is introduced sitting on.
- Internal Reveal: Unazuki wears the same school uniform worn at Rei's T.A. Academy for Girls (which had been established with her a story arc earlier in the manga).
- The Jailbait Wait: Discussed. After learning that Usagi has never formally kissed Mamoru, Unazuki deduces that they're waiting until Usagi is of official age before they do any acts of affection.Unazuki: I mean what creepy, old college man would kiss a fourteen-year-old? I think that's illegal in some places...
- Kissing Under the Influence: Of the magical variety over alcohol. Due to her lack of a Pure Heart, Unazuki develops a one track mind as she sudden wants to obtain her First Kiss by any means necessary. Even if it means kissing the Inner girls who are within reach of her.
- As Mamoru's first kiss with Usagi involved this with alcohol (in the original and Viz dub only; the DiC and Cloverway dub retconned the scene as Serena's dream), it's implied this is why Mamoru was hesitant to tell Usagi how he remembered their first kiss.
- Let's Split Up, Gang!: The Inner Guardians decide to separately search different parts of the city to look for the missing Pure Heart while Rei stays behind to watch over Unazuki.
- MacGuffin Melee: Unazuki's Pure Heart is stolen by Osuji and is lost during her first fight against Uranus and Neptune. Soon Sailor Moon and Tuxedo Mask show up and it becomes a three-way battle to retrieve the heart at the local dump.
- Mass "Oh, Crap!": Usagi and her friends react this way after opening Unazuki's door and seeing her unconscious.
- The Needs of the Many: Uranus warns Sailor Moon that thousands will die if they don't sacrifice the ones whose hearts bear Talismans.

In the Viz Dub, they're more worried about her being kiss-happy with everyone while Minako asserts that the kiss should be reserved for "the person you love".
- Race Against the Clock: The longer Unazuki is without her heart, the more her body begins to fall ill which puts her in a critical condition.
- Reused Character Design: Michiru's Imagine Spot of Adam and Eve resemble Mamoru and Usagi.

- The Self-Cleaning Daimon: Osouji is able to locate her target's Pure Heart through her nozzle.
- Ami pulls up her supercomputer to look for not only Unazuki's Pure Heart but also the Daimon.
- Vicious Vac: The Monster of the Week is Osouji who was born after fusing with Unazuki's elephant vacuum. She comes equipped with various vacuum themed attacks and can track her target with the nozzle feature.
- What the Hell, Hero?: Uranus snaps at Neptune for being relieved that they didn't find a Talisman in Unazuki's Pure Heart.
- You Have GOT to Be Kidding Me!: The exhausted Inner girls are flabbergasted and disgusted that Ami still wants to go through with their study session after everything they've been through today. It doesn't help that she tries justify it.
- Earlier, Rei is also annoyed that Ami insisted on having a study session when they're guests at Unazuki's apartment.
