Major spoilers abound.

  • The game in general provides several soundtracks that manage to convey scary and intense situations perfectly. These include "Phantom Menace" and "Madness" from Part 1, "Twisted Horrors", "The Night of the Dead" and "The Cunning" from Part 2, and "Bleak Future", "House of Corrosion" and "Miasma" from Part 3.
  • Dancing with the Beasts:
    • Anna finds Hotel Reger darkened before discovering that Otto has been tied up in a chair and left to freeze to death in a room with an open window.
    • The Nazi Party has gotten so fed up with Professor Clark's refusals to help them that they've dispatched two assassins to hunt him down. Even after the duo are thwarted, Clark concludes that his only hope is to escape the country.
    • There are Savage Wolves running uncomfortably close to the hotel. Anna and Otto have to save Professor Clark from them, one of the assassins is found mauled by them, and the remaining assassin, wounded in the final fight at the hotel, succumbs to his wounds and is caught by the canines in the middle of a snowstorm. As the wolves surround his body in the ending cinematic, the scene darkens before you hear his dying screams as they're tearing him apart.
  • The Dada Killer:
    • The chapter deals with the titular Serial Killer who murders women to use their bodies in macabre pieces of Dada art and keeps sending Anna taunting messages to get recognition by having her write about the murders.
    • The first victim, Vanessa Fiedler, is not only strangled to death, but her blood is mixed with the paint around her, and she's sexually assaulted.
    • The second victim, Wilma Sommer, is pregnant at the time of her death.
    • When Anna goes to check on Natalie Meisner the day following Gustav's arrest, she's shadowed by the Dada Killer in the staircase and finds the old lady murdered, sitting on the same rocking chair where Anna left her the previous night to cope with her son's arrest.
    • After finding Ruben in the hotel room, Anna and Judit break into the bathroom that's filled with steam coming out of running hot water. After shutting off the water, they see a silhouette behind a curtained bathtub. Removing the curtain reveals the hanged, slashed and almost naked body of Kiki Hag/Nadia Shwetz that falls into the bloody water of the tub.
    • Judit herself is murdered along with Ruben in the latter's cell just when it looked like Ruben was the Dada Killer.
    • The endgame has Anna finding the Knef Gallery covered in disorienting Dada decorations, Iris tied up in a chair, and Ulli unconscious at her feet. The Dada Killer is revealed to be Oskar whose murder spree is motivated by his desire to get back at Iris who refused to exhibit his Dada artistry a second time despite his worship of her.
    • Though Oskar fails to escape and is injured, he laughs and gloats to Anna in order to have her shoot him under the belief that'll make him go down in history as an artistic genius. After Anna denies him the satisfaction, he continues boasting that his true masterpiece is yet to be made before he's taken away in an ambulance. This is followed by the ending cinematic revealing the ambulance crashed, the doctors killed, and Oskar escaped with a bloody message inside the ambulance: "DADA is Not DEAD".
  • Grey Nature: When Carolin first appears, she has a seizure that requires an adrenaline shot to cease.
  • Forgotten Memento:
    • Anna wakes up in a dark cell with injuries and no memory of what happened (which is further enforced by the player's inability to read her journal). She's forced to recall bit by bit what happened in the preceding days. She's soon visited by Oskar who's wearing a creepy paper bag mask over his head and implies he'll keep her alive for as long as he wants to.
    • Suspicious of the number of flies in her cell, Anna finds a crack in the wooden wall. After cracking a piece of board loose, she discovers the dead head of Iris Knef.
    • Gerhard is revealed to have escaped from prison and joined the Villain Team-Up between Oskar and Dhara. He makes his presence known by kidnapping Anna under the pretense of being a taxi driver.
    • The newspaper Oskar inteds to give Anna reveals that Hotel Reger has been burned, and there's no knowledge of Otto's fate as of yet.
    • As Anna tries to hide from Dhara in a locker, her Ill-Timed Sneeze alerts the brute who tricks Anna into thinking she has evaded discovery. Dhara suddenly opens the locker, throws Anna across the room, and lifts her up to strangle her. Though Anna is saved by someone, she still falls unconscious as the chapter ends to be continued.
  • No Quarter:
    • Antoine is brutally cut up by Leon who spreads his body parts across Zürich along with stashes of his money in bags. Leon keeps Antoine's head for himself and talks to the head as if Antoine's still alive.
      • Perhaps more disturbingly, much of the public seems unfazed by the body parts, rushing to find the bags just for the money.
