Pathologic 2's unsettling atmosphere goes hand-in-hand with its sense of humour.

  • The Bachelor calling Artmey 'Vorakh' even after being corrected is amusing. Later in the game, a guard who comes bearing instructions issued by the Bachelor also calls him 'Vorakh'.
  • The text in the mind map describing the various nodes often have an ominous or significance-laden tone. But the update you get after your first conversation with Stakh Rubin:
    Node text: Rubin is an idiot.
  • It's very easy for players misunderstand the surgery mechanic, and accidentally extract not just a shiv, but a heart and brain from Piecework, and then get beaten up by Piecework's friend.
  • The way the game introduces the optional baby-rescuing side objectives. A clerk at the Town Hall assumes that Artemy has brought a baby with him. These are the possible responses:
    Option 1: You thought I brought in a baby?
    Option 2: You thought I brought in a baby?
    Option 3: You thought I brought in a baby?
  • The Odongh that ferry you around town require 'fingernails', a type of coin, as payment. When Artemey is informed of this, the following exchange can happen:
    Artemy: What about toenails?
    Ferryman: We've heard this joke too many times. It was never funny.
  • The Bachelor's penchant for using Latin phrases returns, and Artemy finds it very annoying.
  • Early on in the game, you can have a conversation with one of the Town drunks near the Broken Heart. The drunkard, with much Sesquipedalian Loquaciousness begs you for a drink. Artemy's response:
    Artemy: You're literally next to a pub.
  • This exchange between Artemy and Aglaya Lilich:
    Aglaya Lilich: …I understand. Now please be quiet for… twenty-five seconds. I need to think.
    Option 1: ...
    Option 2: […twenty one …twenty two …twenty three …twenty four…]
  • A lot of Artemy's dreams are meaningful and unnerving in some way, but halfway through the game, he has a dream in which he attends school in Lara Ravel's house and is asked by the Changeling if he can copy his homework, before Lara herself chastises him. The Bachelor stands in the corner throughout the dream. The reason?
    Lara: Daniil was punished for his insolence.
  • Artemy may not be the most responsible adult to have around during a plague.
    (to a kid) "Gorkhon water will turn you into livestock. Saw it with my own eyes."
  • Artemy wants to prevent a State Inquisitor from reaching the town, so he goes to the local mob boss and tells him:
    "I wanna blow up the railroad tracks."
    • And the mob boss instantly agrees that this is a rational response and makes a plan to help.
  • Meta example: The update which added difficulty sliders was accompanied by an announcement video. The video consists almost entirely of developers gushing about how you can now make the game even harder. At the end of the video the interviewer reminds the developer on camera about making the game easier, with the response being a curt "Oh, right."
