Talk about obscure - even being as well versed with the Mythos as a whole, I never heard of this Great Old One. Could be a RPG original, or from something newer. I'll have dig up my references on this.
Edit: Checked out my Call of Cthulhu Malleus Monstrorum - Ut'tulls-Hr'ehr is a GOO from a obscure Mythos novel "Nightmare's Disciple". According to the book, it's cult is a small all-female group that worship this creature via... well I rather not get into details, but I can suspect it's something Yaoi fans would like.

In episode 10, when Nyarko says she gave Mahiro a cruel angel's kiss, was that a subtle reference to Neon Genesis Evangelion.
