- Alternative Character Interpretation: Due to the ambiguous nature of the photograph and Smile himself, many of these have been proposed, from Smile being a Hellhound, to not existing in the first place and simply being a shared delusion, to only being a supernatural Attention Whore who will become docile when enough people have spread the word.
- Dancing Bear: As with a lot of classic creepypastas of the day, the edited photo came first and is known to more people than the story attached to it afterwards.
- Draco in Leather Pants: Popular fanon noticeably puts Smile through the ringer, making him closer to a cute canine Hellhound than the ambiguously real Eldritch Abomination he is in the original. Many fics even depict him as a physical dog, often the pet of Jeff the Killer.
- I Am Not Shazam: Notably, the original story never uses 'Smile Dog' in reference to the creature. The photograph is the titular 'Smile.Jpg', and the creature himself is called 'Smile.dog'. Most fans adopt the name of the story itself to refer to the monster, and similar derivatives.
- Recurring Fanon Character: Grinny Cat, a character originating from Pastamonsters, often depicted as a feline counterpart to Smile with several of the same visual cues and abilities. Fanart often pairs them together, even from creators who have never read the original comic.
