  • Awesome Art: A lot of praise is given to the game's hand-drawn art style, for good reason.
  • Demonic Spiders: The mental patients in the asylum levels. Unlike other enemies, they cannot be outrun or hidden from, if they discover you you will have your throat torn out unless you're right next to a door. To avoid them, you must walk a lot more cautiously and try not to be within touching distance of them, which becomes easier said than done when they switch up their walk cycles, and the space around them becomes more and more narrow.
  • Fridge Horror:
    • While the "Wayward Dreamer" ending ends on a sweet note (Thomas kissing his asleep sister in worry), some players theorize that he is instead dooming himself to repeat the dreams, or actually is trapped in them (since he goes back to bed, the dolls remain desolate, et cetra).
    • The very first thing you see when you start the game is young Gabby being stabbed by Thomas (presumably). Which begs the question in regards to a specific ending: is this how Thomas and Gabriel's daughter died in the "Final Descent" path, with Thomas either murdering her or killing her by accident?
  • Squick:
    • Thomas finding himself in bed with his sister during the "Final Descent" path, in which Thomas is justifiably horrified. Gabrielle tells Thomas that he doesn't have a sister, and that she is his wife, which is actually true in this particular ending route.
    • The meat grinder, with the pulp of blood lying next to it. It's a bit more detailed than most of the other blood splatters. Later in the game it's revealed to be your hand, making this worse.
  • Tear Jerker: The "Final Descent" ending. While the other endings are bittersweet, this ending plays up the bitter: Thomas' nightmares are a result of his wife leaving him, because he can't get over their daughter's death. The fact that the game opens with Thomas stabbing a young Gabby doesn't particularly help.
