"Get off my fucking lawn!" - Helen O. Byrne to Chalithac
Nightmare Fuel: This game is basically one giant oil gusher of Nightmare Fuel, usually with a good dose of Body Horror thrown in.
Angels of The Unsated One - an entity of all-consuming hunger. Two beautiful angels, wings and all, completely soaked in blood, lovingly tearing off strips of each others' flesh and eating it. GAH.
Players once ran to try to interrupt an evil ritual, and arrived too late to find the sacrificial victim with yards of his intestines pulled out his body and strung up into the tree branches. The players had to reach into the intestine-pile (which was made of stuffed sausage casings and fake blood) to retrieve the ritual scroll to determine what happened. One player, who works as an EMT, described the scene as highly accurate.
Early on in the game, some people (a doctor, an emt, a tavernkeep, a bio teacher) went to a nearby town to mercy-kill the residents to prevent them from incurably becoming ghouls due to cannibalism. Not only were there real OOC nightmares, but there was real OOC throwing up afterwards. For several of the folks who went, that was a character defining experience.
Demonic Spiders: The green soldiers, which can nail you on their first shot, the grenadiers, whose grenades aren't indicated with a red flash unlike all the other projectiles in the game for whatever reason, the machine gunners, and the launchers, whose three-round burst can lose your health if you're not paying attention.
Game-Breaker: The cane gun. Not only can it instantly recoil enemies, but it also has infinite ammo unlike the other weapons.
Narm: The voice acting is so terrible that they'd make you cringe, including Westermann once he falls off the cable car. Not to mention some of the death animations in the main game, and the narrator in Mighty Joe Jupiter.
Special Effects Failure: If one looks closely, the dropship has a superimposed explosion special effect when the cable car falls on it, giving the impression that it looks cartoonish.
That One Attack: Octavian has him firing a projectile that can't be dodged. You have to shoot it down before it can hit you.
That One Boss: Tansi and Tori. Tansi moves very fast and she can surprise you by appearing right in front of you and slashing you with her sword if you're not quick enough. Tori is also very hard to hit and she can come out and take lucky shots at you if you're not very careful. The same goes for Westermann, whose shotgun blast can hit you if you don't hide in time, especially if it's your first time playing.
