Nowak was conscripted to fight in World War II.

There's a lot of elements in the game that allude to warfare in general (the Conscription Notice item, various other items representing military equipment and battlefield surgery, Boyle and Griffin, and Hickory), while Trigger Fingers is all but stated to be either a Nazi officer or Hitler himself. Given that the whole game is Nowak's dying dream, with various elements being pulled from his psychological hang-ups, it seems very likely that he was directly involved in the war.
