J.D. and Carla struggle as cracks appear in their friendship. Turk battles Kelso for a prime lunch spot and Cox and Elliot disagree over the discharging, or not, of a patient.
- Jerkass Realization: JD hurts Carla because they were close friends and their evolving professional relationship meant he had to stand up and be the Doctor to her Nurse. This plus Cox being Cox brought them into conflict.
- "Not So Different" Remark: Cox finally gets through to Eliot that the reason she's going out of the way for her patient because she sees herself in her patient's struggles.
- Odd Friendship: JD and Carla try this, but it doesn't quite work, because they work together and have different backgrounds and roles at the hospital.
- Sad Clown: Eliot's patient won't stop talking because she's trying to keep the emotions at bay. Then Cox calls Eliot the same.
