The Saradominist monks on Harmony Island have a problem, and it involves zombie pirate mad doctors. Some undead surgeon called Mi-Gor has transplanted their brains into zombie bodies, and they want you to help them get their grey matter back where it belongs, enlisting Dr. Fenkenstrain (the Dr. Frankenstein parody from Creature of Fenkenstrain) for the surgery itself.

First, you're going to have to solve a more pressing problem- spying on Mi-Gor and his minions has revealed that they're covering the island with sleeping gas to get the monks holed up in the granary- it won't affect the zombies, but the monks have living brains and so will be knocked out, at which point Mi-Gor can reclaim them for his experiments. In this case, you're going to have to borrow the power of Saradomin to protect the granary, by using a prayer book from the Edgeville monastery. Once the situation is no longer time-critical, it's time to fetch the good doctor.

Fenkenstrain is rather easy to convince, because when you last met him, you stole his Ring of Charos. The Ring was enchanted to increase the wearer's persuasive skills, and he used it to convince the xenophobic werewolves of Canifis that he was one of them. Without it, he's locked himself in the topmost tower of his castle in fear of them, and it's only a matter of time before they attack. Getting him out is more of a problem, as he's somehow allergic to teleportation. So instead, you hide him inside a box of meat for Rufus, owner of the Canifis meat store, to ship to Mos Le'Harmless.

… And then the whole 'avoid teleportation' thing becomes pointless, since Rufus uses teleportation magic on his shipping orders. Oh well, at least Fenkenstrain didn't die and can perform the surgery, but he needs supplies. You can get them by killing the sorebones who helped Mi-Gor perform the first operation, but you're going to need diving equipment as the island outside the granary is still covered in sleeping gas, and a keg of gunpowder to break through the front doors.

The surgery itself is covered up by a cutscene of a kitten playing with yarn, but the Brain Bleach is still very necessary. Then you go kick Mi-Gor out of the monastery, fighting his secret weapon Barrelchest, a zombie pirate robot.

Along the way, you'll find out that Mi-Gor, like Donnie, is working for Rabid Jack. This guy seems to be a problem, you should probably investigate him...
