In 2021 Ethan and Mia winters are living in Romania with their daughter Rose. One day Chris Redfield and Hound Wolf Squad break into their house, gunning down and seemingly killing Mia before kidnapping Rose and Ethan.
Ethan awakes near the wreckage of a vehicle with Rose missing. He follows a trail that leads him to a remote Village. After witnessing the death of a local villager and fighting off a hoard of Lycans Ethan meets an old hag who tells him that Rose is in the Village.
Ethan meets a group of survivors hiding out in a building. They are abruptly slaughtered when one transforms into a Lycan leaving Ethan on his own again.
Ethan is then kidnapped by a man with magnetic powers named Heisenberg who brings him before Mother Miranda and the other lords. Ethan narrowly escapes and outside meets the Duke, Who sells him weapons.
Ethan enters Castle Dimitrescu and kills one of the lords named Lady Alcina Dimitrescu along with her daughters and takes a mysterious flask.
He then meets the hag again who tells him that Rose will be sacrificed soon.
Ethan meets up with Duke once more who provides an info dump explaining that the four lords each have one of four flasks which contain the dismembered body parts of Rose. The Duke assures Ethan that Roses essence is still intact and she can still be saved.
Ethan then enters House Beneviento where he is forced to solve puzzles, witnesses strange seemingly ghostly phenomena and is chased by a giant deformed Fetus monster. After stabbing Donna Benevientos seemingly sentient doll Angie several times with a pair of scissors he suddenly finds Donna on the floor dead in Angies place. On the way out Ethan can find notes explaining that the ghostly phenomena Ethan encountered was actually hallucinations from a fear gas like toxin produced by Donna. Ethan takes Donnas flask.
Ethan then heads to lord Moreau’s reservoir and encounters Chris and his men, but they are separated when Moreau attacks in his mutant fish form. Ethan kills Moreau and takes his flask.
Ethan then receives a message from Heisenberg who leads him to a stronghold. Ethan takes the last flask and places all of them in the village Altar.
