Tommy begins a search for Tim’s replacement, but they have trouble finding a good one. Paul’s sister visits.

  • Conjoined Twins: Grace deals with a pair of these over a 911 call.
  • Cowboy Cop: More like “cowboy firefighters”, but Pearce remarks on seeing the traits of this trope in the 126.
  • Eye Scream: A bicyclist falls face-first onto his reusable straw, which goes through his eye.
  • Innocently Insensitive: Owen explaining that the reason he finally decided to schedule his tumor removal surgery is the prospect of becoming a father to Gwyn’s unborn baby clearly stings for TK.
  • Internal Reveal: Offscreen, Owen and Gwyn let TK know about the latter’s pregnancy.
  • Jerkass Has a Point: Pearce outright refusing to enter the minefield may have been the selfish choice to Owen and Tommy, and potentially deadly to the wounded older brother on the call, but he cites common sense and the differences in the spheres of influence between the 126 paramedics and the Austin bomb squad, which is a salient factor. Luckily, the situation is resolved once TK volunteers himself to help his father, citing his paramedic certifications.
  • Land Mine Goes "Click!": Averted. When the owner of the homemade minefield steps onto one of his own mines, there is no "click", he just goes boom!
  • Ludicrous Gibs: The minefield owner gets blown into pieces after stepping on one of his own mines.
  • Never Got to Say Goodbye: Naomi feels that she didn’t have a chance at closure with her sister (Paul’s pre-transition self), and thus never felt that she and Paul were the same person afterward.
  • Red Herring: Mrs. Strickland’s heart health has been brought up between Paul and Naomi a handful of times. When Paul later gets a call about his mother being in the hospital, he rushes there but finds her alive and walking around— the real bedridden one is Naomi.
  • Stunned Silence: TK’s initial reaction to his mother’s pregnancy.
  • Terrible Interviewees Montage: Tommy and Nancy go through three different replacements for Rosewater. Hall is The Klutz who drops an oxygen tank and sends it flying and at one point backs the ambulance over Nancy's foot, Goetz faints at the sight of blood, and Nadler headbutts a patient's boyfriend for getting too close while they're trying to save her life.
    • The final candidate, Pearce, looks like a good fit and has a perfect record of saves, but it quickly becomes apparent that he only has that perfect record because he won't put himself at any risk whatsoever. This does not fly for Tommy, since their job inevitably requires them to go into risky situations to help people. Though his passive-aggressiveness when he brings it up doesn't help.
  • Use Your Head: Nadler, one of the paramedic recruits, headbutts a civilian for not backing away. She's just as hurt from the headbutt as the civilian is.
  • The World's Expert (on Getting Killed): The minefield owner says he knows where every mine is, only to step on one and get blown up.
  • You Called Me "X"; It Must Be Serious: Naomi has always referred to Paul as P since his transition until she makes up with him, finally calling him by his name.
  • You See, I'm Dying: Paul finds out that his sister, Naomi, has had multiple sclerosis for three years.
