Crossovers
  • Weiss' frequent praising of Pyrrha is this in BlazBlue Alternative: Remnant. As seen through Pyrrha's POV, Weiss drawing attention to Pyrrha's accomplishments makes her rather uncomfortable since it essentially only acknowledges her as the "Invincible Girl" rather than as a person, even if the praise means well.
  • In Cardcaptor Rad, Hot Shot manages to both get High Wire mad at him and depress Rachel when he counters Optimus' praise that Rachel did good with protecting High Wire from the Decepticons.
  • In Contact of Races- the sequel to Contact at Kobol- the Tau'ri initially anger the Opochti by referring to their world as Vantic 5, but the Opochti accept their apology once the Tau'ri representatives clarify that they just didn't know what the natives called themselves and were relying on the databases taken from the Race.
  • The Doctor is often tactless when it comes to taking human feelings into account, but in Dangerous Tenant he almost takes this too far when he nonchalantly ‘introduces’ Claire Redfield to the mutated Steve Burnside and only remembers after he's made the introductions that Steve was Claire’s boyfriend before he was infected and mutated.
  • The Dragon and the Butterfly Saga:
    • The crux of the conflict in Chapter 6 of Dragons, Butterflies, And Who Knows What Else? is that Hiccup unwittingly upsets Antonio with the idea that everyone will die eventually, Hiccup having been taught such things at a much earlier age (as were everyone else on Berk).
    • The Dragon and the Butterfly: Whiteout:
      • Karla's Super-Speed makes her rather brash and cocky.
      • While Pedro was flighty before, ever since he got his gift he seems to be less aware of his surroundings than usual. In one instance, he draws a waterfall while the entire family is waiting to go to the bathroom.
      • Peep's Safety Freak behavior tends to reach into this territory, often micromanaging everyone's behavior in order to "protect them" from any potential (usually imagined) dangers she comes across. The fact that she blows a whistle whenever this happens only serves to grate on everyone's nerves.
  • Cultivating Empathy: While undressing Xichen in order to treat his injuries, the Healers almost take his forehead ribbon off, unaware that only a Lan's spouse is allowed to touch their headband. Gala has to read his memories in order to understand what's wrong and defuse the situation.
  • Equestria Girls: A Fairly Odd Friendship: Sunset tries to apologize to Moondancer for having bullied her years ago. However, her claims that she was "in a bad place," inability to admit any wrongdoing, and running off in the middle of it to get a text from Moondancer just aggravates her.
  • Ere we go, Pluz Ultra!: When Hitoshi tries to give his canon declaration of war, Izuku first ignores him then confusedly asks where he came from when Hitoshi yells at him. This is because Hitoshi has purple hair which, due to how Izuku's quirk works, makes him extremely stealthy to Izuku. Naturally, Hitoshi interprets it as Izuku thinking him "not important enough to listen to".
  • Harbinger (Finmonster) (Danny Phantom, ParaNorman): Dash innocently makes Courtney uncomfortable by calling Norman weird in chapter 6, and again when he tries to defend Mitch and Scott from a group of homophobic jerks. He says that Courtney is twice as hot as their girlfriends.
  • Due to having spent the entirety of his life in the Jedi Order (and not being a very worldly jedi at that), Master Koth in A Horse for the Force manages to really shove his foot in his mouth when talking to Ranma.
    Ranma: I could even be raped in this form. Something, something which I've had some nightmares about.
    Master Koth: Fear leads to the Dark Side. You should face your fears.
    Ranma: We're gonna have to agree to disagree on that one, you insensitive fuck.
  • In The Marvelous World Of DC, Thor gives Dr. Strange a Crushing Handshake when he first meets him. In Thor's defense, he didn't know about Strange's car accident that damaged his hands.
  • Basically Robert Baratheon's Fatal Flaw in A Song of Ice, Fire and Heart:
    • He was utterly head-over-heels for Lyanna but never even believed she would be disgusted by his taste for loose girls and drinking, blatantly showing off until she ultimately ran away with Rhaegar Targaryen in an attempt to break their betrothal. Vanitas gleefully points Robert could have avoided that if he had been more considerate and actually bothered to listen to Lyanna when she complained about his behaviour.
    • He smiled when presented with Elia Martell's butchered corpse and claimed her young daughter Rhaenys deserved to die for being "dragonspawn", right in front of Ventus who was a dear friend of Elia and was so infuriated by Robert's disrespect for the poor Princess that he punched Robert's face and stormed out of the Red Keep and Westeros entirely.
    • Following Ven's disappearance, Robert wants to make amends and honour his memory, so when he meets Ven's assumed son Roxas, the King decides the boy would be the perfect bridegroom for his own daughter Myrcella, ensuring he will live in comfort and be respected as a Prince of Westeros. Roxas gets pissed when Robert declares the betrothal effective without even asking if the teen wants to get married to begin with, and it causes a rift between him and the King.
  • In the Time Break Saga, Lambda tries to cheer Riku up by scanning Darkness and letting him know that it's not evil, just a form of energy. This just makes him feel worse, as it means he wasn't Brainwashed and Crazy when he did all those horrible things and that everything really was his fault.
  • In The Totally Amazing Spider-Man, Peter Parker brushed off a friendly conversation with Sam, due to his shyness, and a study date with Clover, because he feared she was an Alpha Bitch. The spies, however, thought he was being a pretentious New Yorker. Thankfully, they are able to settle their issues pretty quickly.
  • The Unwavering Light of Calamity: When Lucia tries to stop Wartwood's shipping war over Love Choice in its tracks by suggesting getting rid of the female character (whom she found utterly unlikeable) and just getting the two males together, the town tells her that she's being stupid because guys can't date each other. This causes the closeted Lucia to assume they're all homophobic, and she angrily chews them all out before stomping off; it's only later that she learns that Amphibia (or at least the provincial Wartwoodians) don't have a concept of homosexuality and literally didn't understand how two guys can be romantically involved. Once it's explained to them, they apologize for upsetting her (and also agree with her assessment of the pairing).
  • In Wolverines, Wendigos and Winchesters, Mr. Moore energetically describes the "mutant menace" to Scott, Logan and Dean - members of the staff at Xavier's. Talk about awkward...

Aladdin

  • In Queen of Diamonds, Fatima (Jasmine's handmaiden) and the Sultan each initially assume that Jasmine will be glad about Aladdin's death after he spent two years "forcing" her to stay with him, although once she explains the truth they both accept her view of Aladdin as a good man.

Animorphs

  • Eleutherophobia:
    • In Back to the Future, Aunt Leah and Uncle Joe casually ask Tom why he got infested, not realising that it was an incredibly traumatic experience.
    • In Ghost in the Shell, Taylor calls Tom "Visser Seventeen" when she spots him at a Matter Over Mind meeting because she doesn't know his actual name. Since he hates being mistaken for his Yeerk, Tom doesn't take this well.
    • In The Thing from Another World, Tom is annoyed when Cassie says that true parasites don't harm their hosts, since he spent three years being mentally harmed by Puppeteer Parasites.

Arrowverse

  • Girls Like Us: After Iris asks Laurel to speak to The Flash on her behalf, Barry manages to do this twice. One of them is repeating Oliver's belief that "guys like us don't get the girl". Having been "the girl" herself, Laurel is less than amused. The real thing that angers her however is the revelation that Oliver's been training Barry as a vigilante, something he has absolutely refused to do with her.
  • To Hell and Back (Arrowverse);
    • Oliver and Laurel don't mean any harm when they flirt with each other, but it's only once they push him away for good that they realize that doing it in front of Tommy, whom they both know is still in love with her, wasn't something they should have done.
    • Cisco asks Barry to give him some more work so that he has an excuse to miss his family reunion and not have to deal with his parents and brother. Barry reminds him that his own mother is dead and his father is away from them making up for the lost time in prison, so he's not getting any sympathy from him.
    • When Patricia Arias tries to use Kara as an in to reconcile with Sam, she makes the mistake of asking "who's Ruby?", unaware that she's asking about the identity of her own granddaughter. It pisses Kara off and gets her thrown out of the building.

The Casagrandes

  • Zoo Coup Riot: When Carl meets Pat for the first time, he's confused by her father calling both her and Nat girls, and questions it. Unbeknownst to Carl, Pat is a transgirl and somewhat upset by his comment.

Danganronpa

  • Blackened Skies:
    • While trying to learn whether guilt becomes easier to cope with over time, Gonta unintentionally insults Kaede by asking if she still feels guilty about accidentally taking a life during their first mutual killing game.
    • In a somewhat ambiguous case, Monodam tries to calm down a group of distressed students by offering them free food from the 'Death Row Diner'. All of the dishes are based off of their various executions, and the one he chooses is the latest addition to their menu, reminding them all of what befell the first culprit of their game.
    • When Kaede notices a motorcycle stunt cage among the attractions at the 'Carnival of Carnage', Kaede points it out to Mondo, asking if he'd like to try it out. Mondo freezes up at the sight.
  • Danganronpa: Galactic Melancholy: Angie frequently fails to recognize when her comments are making others uncomfortable. This tragically costs her her friendship with her 'Sista' Akane when she offends her this way and fails to make up with her before Akane's death.

Danny Phantom

  • Resurrected Memories: Due to Ember Becoming the Mask as Amberline, she ends up unintentionally shutting Kitty out of her life for a while. On the flip side, it takes Kitty a very long time to figure out that Ember has truly come to love Danny and that causes her to act insensitively as well.

DC Animated Universe

  • When battling Killer Croc in A Little Less Conversation, a Little More Action Please, Flash does his usual mid-fight banter, only to leave Croc slavering with rage over it due to Flash accidentally opening some old wounds. Afterwards, Flash apologizes as he's used to exchanging banter with his rogues.
    • Earlier, Barbara unthinkingly assigns Cassandra Cain to read a bunch of files, not knowing the girl in question can't read.

Dragon Ball

  • Dragon Ball Z Abridged: During the Cell Games, when Gohan is the only one present who can keep up with Goku and Cell's fight, Krillin remarks "Huh, Saiyans. Am I right?" Then he realizes that he's talking to Trunks and says "Aw shit, was that racist?"
  • A Simple Wish: Gohan unintentionally upsets his mother by telling her that he'd rather go to Videl's Christmas party than spend time with his family that Christmas Eve.

Encanto

  • In Reflections in a Cloudy Sky, Isabela offers Camilo an arepa off her plate, not realizing that he hadn't eaten much because he was feeling sick to his stomach. Camilo eats the arepa anyway to deflect suspicions from Alma, which just makes him feel worse.

Fire Emblem

Friends

  • Monica demonstrates a minor example of this in "The One Where the Chick & the Duck Go to the Farm"; after the deaths of the birds, she immediately comments that she could make a nice meal from both of them, but when Chandler objects to the idea of eating his old pets she swiftly apologises, justifying it as her being a chef.

Grandmaster of Demonic Cultivation: Mo Dao Zu Shi / The Untamed

  • Impenetrable Walls:
    • Lan Xichen doesn't seem to be aware that people are too afraid or awed by his person to not obey him, so his acting as if Wei Wuxian becoming a concubine is a willing choice is more thoughtless than anything.
    • Lan Wangji tells Wei Wuxian he can write a letter to Lotus Pier and Jiang Cheng would have no choice but write back. Wei Wuxian immediately clams up and begs him to leave the matter alone.
    • Wei Wuxian's fascination with dual cultivation is rather painful to witness for Lan Wangji due to Wuxian's obliviousness.
    • Also, Wei Wuxian trying to be a properly obedient concubine reminds Lan Wangji they're not equals anymore — Wei Ying now is his Sex Slave, unable to give him the emotional intimacy he craves so much because it wouldn't be respectful towards royalty.
  • In For What, For All But Myself, Jiang Li, a very little girl, notes at the end of the fic that Jiang Cheng's son is very small, and having some understanding of how hard of a time Jiang Cheng had attempted to comfort him. Unfortunately, she attempts to do so by telling him that she knows he'll do better next time. Given the awful, traumatic circumstances of carrying his child (and the fact that, unless they find another river spirit, he’s not biologically going to be able to!), it bothers him.

Harry Potter

  • The Choices That Make Us: Muggle missionary Brother Allen (who isn't in on The Masquerade) doesn't know any better than to refer to an orphan showing uncontrolled magical abilities as a "poor child", but the mere fact that he does so is a sign that she needs to be raised in the wizarding world, where people see her magic as a good thing.
  • Hermione in Hella Potter and the Reincarnated OC used to lecture Hella on "eating like a pig" because she'd never faced starvation like Hella had, who'd grown used to eating as much as she could whenever the opportunity arose and saving more for later, never knowing when her next meal would come. It ended up being part of the reason Hella remained underweight and stunted in her height as she was too embarrassed to actually eat until she was full.
  • The Very Secret Diary has a darkly hilarious example, mainly due to the person on the receiving end of the unintentional insult 100% deserving it. Tom asks Ginny about this "You-Know-Who" fellow she keeps mentioning, especially when she reveals his name is Lord Voldemort. Wanting to know how his future self is doing, Tom presses her for details, and is at first quite pleased with the results... until Ginny cheerfully explains to him how Voldemort was defeated eleven years ago by the wonderful, heroic Harry Potter, whom nearly everyone in the wizarding world considers a hero. And this happened when Harry was a baby. And how, ten years later, he returned (via drinking unicorn blood and sharing a body with Quirrel), but only briefly, before he got defeated again, by Harry and two other first years. You can practically see Tom thinking, "Oh, come on! Really?!"

Hetalia: Axis Powers

  • Gankona, Unnachgiebig, Unità: Italy is this. Although Germany and Japan made their feelings for him incredibly obvious, he kept on referring to them as "friends". Subverted in that Italy knew all along. He was in deep denial because he thought such wonderful people like Germany and Japan could never love a guy like him and if they did, they would soon "wake up".

Historical Fiction

  • Sister Floriana: In The Birthday, the titular character and her friends try to break the jaded Sister Diana out of her shell by giving her a surprise birthday party, having been told by her best friend Sister Emmie that she misses having them. To their shock, Diana hates the party, and gets annoyed at them for moving her books to make way for decorations. When Floriana follows her however, she learns that Diana's mother is bed-ridden with Polio (over 20 years before there would be a vaccine) and that the reason she missed surprise parties is because her mother always gave them to her. Flori helps cheer her up by praying together for her mother's safety.

How to Train Your Dragon

  • In Dark Valkyrie, Tuffnut falls into this when he becomes curious about the scars Hiccup got during his torture. He gets an accidental elbow to the face from Hiccup and a series of scoldings from Heather, Ruffnut, Snotlout and Fishlegs for the trouble.

Invader Zim

Kim Possible

  • In "Equal Romance 02: Obscure Ways to Devotion", while Kim is exploring Ron's mind to force him out of a coma, she realises that she once inadvertently did this to him when they were six years old and she found Ron crying after they'd been playing a game earlier. At the time, Kim started teasing him because she assumed he was only crying because he lost the game. As Kim learns while witnessing the full memory in Ron's mind, he was actually crying because he'd just overheard his parents admit that they never actually wanted him and was naturally devastated, leaving Kim even more shaken that she unintentionally added to Ron's poor self-esteem issues at that time.

The Loud House

  • A Loud To Stay: After Lincoln learns that he was adopted, Lucy asks him to listen to her recite her new poem, "Outsider". She has no idea about the situation, or that her poem makes him even more anxious about how his sisters would react if they learned the truth.

Marvel Cinematic Universe

  • While interrogating the Other Clint in The Others, Rhodey gets tired of having to repeat himself so many times and sarcastically asks if he's gone deaf. Clint responds that yes, that tends to happen when a flash grenade goes off next to your head.

Marvel Universe

  • A Prize for Three Empires: When Carol Danvers is introduced to the new Captain Marvel Monica Rambeau, she jokingly asks if Mar-Vell has retired or gotten a new look. Her shocked friends inform her that Mar-Vell got sick from cancer and died.
    She was a black woman in a black-and-white costume, and, when she chose to, she was composed of light. The Thing introduced her as the latest Avenger to the three, Nick, Logan, and Carol, who hadn't met her. "Carol Danvers, meet Captain Marvel."
    Carol dropped her jaw, and then asked if Mar-Vell had retired, or suddenly gotten a new look? She meant it as a joke.
    It wasn't taken as such.
    "Carol, didn't you know?" asked Nick Fury.
    "Know what, Nicholas?" she responded.
    "Mar-Vell's dead."

Metamorphosis 2013

  • A Series of Fortunate Events: Saki Yoshida goes out with her friend Chinami shopping. Chinami sees a beautiful and revealing dress and suggests Saki gets to dress for herself. Chinami, unaware of Saki's past of being a prostitute and in abusive relationships, does not notice how uncomfortable Saki is with the dress because of how revealing it is. Saki does not hold it against Chinami and shares her discomfort with the dress without revealing her past; after talking with Chinami about her discomfort with how revealing the dress is, Saki decides to get the dress for herself, only for her to realize that the dress is too expensive for her.

Miraculous Ladybug

  • Cheshire: Max gets very upset when Cheshire shows up wearing the Horse Miraculous in front of him, Misterbug, and several other members of his class. When he confronts her about it, Cheshire defends herself by pointing out she wasn't aware he was in the group, only to accidentally let slip that he hadn't treated Kaalki well anyway.
  • Couturiere: Adrien combines this with Didn't Think This Through; he is so laser-focused on trying to keep Lila from getting akumatized again that he completely neglects Marinette's feelings, ignoring how Lila is isolating and breaking down Marinette until it's too late to prevent her akumatization.
  • Feralnette AU: Alya is prone to this, courtesy of her Skewed Priorities and lack of a verbal filter. For instance, she completely fails to understand how filming somebody for her Ladyblog without their permission might be a problem, and when she notices that Marinette has hit it off with Felix, immediately asks her if she's only befriending him in order to get closer to Adrien.
  • The Karma of Lies:
    • Adrien doesn't see anything wrong with Lila tricking her friends into donating to a Fake Charity, since his family is so wealthy he doesn't understand the value of material goods. When Plagg points out that not all of his classmates can necessarily afford to replace the things they've been conned out of, he blows him off.
    • He also dismisses Marinette being hurt by them donating the outfits she worked so hard to make for them, dismissively declaring that "They're just clothes" and that she can just make more. His complete lack of regard for the time, effort and care she put into them sends her into a Tranquil Fury.
  • Leave for Mendeleiev: When Audrey remarks to Marinette that "My daughter could learn a lot from you," Marinette flinches. Audrey isn't aware that said daughter has been bullying Marinette for years, or that their teacher constantly pressured her to 'set a good example' for Chloe by constantly forgiving every trespass and letting her walk all over her.
  • In Manynette Sodium Rich Edition, Kim recruits Lila for his plan to draw the attention of all the akumatized Marinettes with a fashion disaster. Mainly because he believes she's an expert on fashion disasters, given what she normally wears. No insult is intended, even as he casually tears down her whole look, comparing it to a rant Marinette gave him after seeing him in Naruto cosplay.
  • Adrien in Nymph and the Corrupted Miraculous comes across as this to Marinette when he implies that she can't do things as simple as getting to her seat because she's handicapped.
  • In A Small but Stubborn Fire:
    • Sabine points out to Marinette how Paris seems to revolve around her with all the connections she has made. This hits a little close to home for Marinette as Ladybug with the pressure on her to keep the people of Paris safe.
    • When Sabine makes a joke about Marinette love life, and how Cat Noir seems like a playboy, not realizing that her daughter is Ladybug while knowing for well that Cat is in love with his partner, Marinette doesn’t respond to the teasing.
    • Even after being grounded, Marinette tells her mother that she loves and trusts her, right after Sabine came so close on whether or not to violate her daughter's privacy and read her diary. Sabine feels so ashamed that she has to go to her room and cry.
    • After the incident above when Sabine tells Tom she is going to talk to Nathalie and he tells her that he will trust her to handle it, those words hurt Sabine.

Monsters vs. Aliens

  • God Help the Outcasts
    • Link, when he gets a close look at Susan's face for the first time, makes a comment about how he never realized how unattractive human faces were. Susan, who's already feeling insecure about her appearance due to her transformation, runs off crying. Link honestly didn't know better, and looks/acts guilty for a while after.
    • After Susan is shot in the leg due to a misunderstanding, Dr. Cockroach tries to treat it lightly by calling it her "first angry mob" and saying that it simply comes with being a monster. Needless to say, this is not what Susan needed to hear right then.

Mork & Mindy

My Hero Academia

  • Several underground heroes in Deku? I think he's some pro... unthinkingly call Izuku "Deku" all the time because when they'd first asked his name, Izuku had had a particularly rough day and repeated Bakugou's insulting nickname for him. Once they realize what "Deku" actually means, they're all horrified and offer their help to change his hero name (by this point, Izuku was moderately famous in the hero community as "Deku"), but Izuku turns them down, citing that it doesn't feel like an insult when they say it.
  • Entropy: The Fate of the Hero System: Izuku Midoriya, due to his personal belief that Momo is a great hero, refuses to accept that she is having problems with her hero career.
  • Erased Potential: Toshinori bumbles headlong into this when he takes an interest in Izuku... after Izuku has already spent the better part of two years training under Aizawa to become a Quirkless hero. When they learn about his intentions, Aizawa is coldly furious, while Izuku struggles with being unintentionally insulted by somebody he's long idolized.
  • Mastermind: Rise of Anarchy: Aizawa runs into one of his former students — one he previously expelled from U.A. after deciding he didn't have what it took to make it as a hero. He's since become a vigilante, and helps Aizawa out of a tight spot. Aizawa wishes him luck, stating "I hope your second try goes better than the first." Given that he was the one who ended that first attempt, the sentiment doesn't quite hit the way he intended it to.
  • In Metallurgy, both Setsuna and Shinso accidentally make a particularly insensitive comment about the USJ event. Setsuna comments that it's impressive Class-1A faced so many villains and came out unscathed while Shinso comments on Izuku getting an extra week off classes as a "perk of being Class-1A's president". In reality, Izuku lost a hand in the attack and the week off was physical therapy to get used to his new prosthetic (made from his own Quirk).

My Little Pony: Friendship Is Magic

  • In Brother Bothers, Pinkie Pie is this to Cheese Sandwich and his brother Tomato concerning their nasty Sibling Rivalry. She forces them into a "Brotherly Bonding Time", not realizing that it would only aggravate their already tense feud, up to the point where both of the brothers end up exploding at each other. She realizes this at the end of the fic and apologizes to them.
  • In The Dear Sweetie Belle Continuity, Rarity had just found out she was pregnant after a party her roommate had dragged her to. Said roommate sees her crying and asks her if she wants to go to another party. Rarity freaking snaps.
  • The Demesne Of The Reluctant Twilight Sparkle: Upon learning she's got a new assistant, Twilight asks her "Could you fetch me a cup of coffee?" When the mare glares and angrily says "Fetch?", Twilight responds with "Oh, don't make me beg." The mare becomes even more offended, tells her to go deal with what's happening outside on her own, and storms off. After dealing with the crowd outside, Twilight comes back in, talks to the mare to try and figure out why she's so mad, and finally mentions that she doesn't even know the mare's name. It turns out to be Golden Retriever, and she's heard so many jokes about it that when Twilight started saying things like "fetch" and "beg", Golden thought she was starting in too and lost her temper. Fortunately, once they both realize their mistakes, Golden calms down.
  • A Diplomatic Visit: Equestria in general, as it and a great deal of its people tend to steamroll over those of other nations without considering their feelings. Averted by certain hate-groups who are deliberately opposed to any non-ponies.
  • Her Inner Demons: The Shadowbolts, thanks to their hyper-competitive nature didn't realize they were aggravating Twilight. It becomes Played for Drama, since Twilight had already been bullied and ridiculed, and their own actions only drove Twilight into deeper despair and allowed her darkness to consume her and become Midnight Sparkle, and they themselves end up feeling horrible for ignoring Twilight's emotional breakdown
  • In Hopes and Follies, Twilight assures Trixie that she doesn't have to worry about their past conflicts, because nopony holds her responsible for it and they hardly even remembered the Ursa incident until she came back. It's not until after Trixie storms off overwhelmingly pissed that Twilight realizes that a professional showpony would want to be remembered, even as a villain. Not to mention that those events were some of the most defining moments of her life. And Trixie has an overwhelming need to be recognized by Twilight, one way or another. This results in Trixie resolving to become an enemy, rival or problem that Twilight can't ignore... ironically, while Twilight's resolving to become closer to Trixie and accept her as a friend.
  • Loved and Lost: In the Season 2 finale this Accusation Fic was based on, the fact that the original bridesmaids were imprisoned and replaced is only briefly touched upon, so the fic rectifies that by bringing this trope into play, with the bridesmaids being among the outraged Canterlot citizens who shame the heroes after Jewelius pins the blame on them for the invasion happening. Even though the Mane 5 genuinely didn't know that the bridesmaids were imprisoned, they're soon forced to acknowledge that they were too excited by the wedding to be concerned for their disappearance, as well as dismissing Twilight when she tried to warn them that something wasn't adding up. When called out on this by the bridesmaids, Shining inadvertently rubs salt in the wound by telling them the excuse Chrysalis had given him (firing them for only wanting to meet Canterlot royalty). An upset Twinkleshine starts crying while expressing disbelief that Shining believed that lie, which only angers the crowd who sympathize with the bridesmaids even further, and when Twilight responds to Shining's protest that he hadn't meant to hurt her feelings by saying that he still did, just like when he dismissed her in the wedding rehearsal, he has no further rebuttal. Thankfully, once peace is restored in the end, the heroes make up for this specific wrongdoing by apologizing to the bridesmaids and offering their original positions back, upon which the bridesmaids forgive them and allow them to keep the positions as an apology for spurning them under Jewelius' reign.
  • The Pieces Lie Where They Fell: Vix-Lei's humor tends to be this way, as she doesn't realize her jokes might be offensive to others. At least, at first - she does recognize that the others don't necessarily find them funny, but jokes are the best way she knows to break the ice.
  • RainbowDoubleDash's Lunaverse: At the Grand Galloping Gala, Night Light tries to brush off his behavior towards Trixie when talking with Ditzy by condescendingly telling her she doesn't know what it's like to worry about children (since an argument between Twilight Sparkle and Trixie led to Twilight going on the run). An unoffended Ditzy tells that she is a mom, actually, and does know. Night Light immediately apologizes for his remark.

Naruto

  • Space to Breathe presents this as Naruto's Fatal Flaw. While he cares about Sakura, he's also caught up in his idea of her, convinced that he's competing with Sasuke for her affection. He unintentionally helps set the events of the story into motion when he dramatically misunderstands why she's so concerned about entering the Chunin Exams, suggesting that she's worried about her appearance rather than everyone's safety. Later on, when he runs into her for the first time since said Exams, he immediately asks her out for ramen, remarking that "it can be a date now that you're not interested in Sasuke!" It takes Sakura lambasting him for Loving a Shadow and not making any effort to get to know the real her for him to realize the part he played in driving her away from Team Seven.

Neon Genesis Evangelion

  • In the Peggy Sue fic The Second Try, Misato jokingly makes a comment that Shinji and Asuka should use protection when she thinks they want to stay home to have sex. Not the best thing to say to two parents who's daughter was just Ret Goned.

Odd Squad

  • OSMU: Fanfiction Friction:
    • In Chapter 4, when Oswald is suddenly teleported from the Shower Room to the Lab while naked and in the middle of taking a shower, Opal, who is deliberately avoiding looking at Oswald like the other three girls, asks Omar how Oswald is doing. He responds by saying that Oswald's stomach is moving up and down rapidly from hyperventilation...just before remarking that his stomach moving like that is cute. This earns him a glare from Orla.
    • The same chapter has Orchid giving Oswald some choice words once the ordeal is done and over with. Of course, since this is Orchid we're talking about, her being insensitive isn't unlike her.
      Orchid: Hey! Pinky! Next time you and your buddies go streaking, y' might wanna clean up after yourselves! I went in to return a gadget and nearly killed myself slipping on some water!
      Oswald: Sorry... [as Orchid walks away] Of all the people in the universe...this day can't possibly get any worse...
    • Todd is hesitant about being The Mole for the Mobile Unit and going to sniff out Basil Valentine to find out his intentions. Opal, being the one who is on a quest to get a fanfic for the Mobile Unit written, remarks on how great of a fanfic it's going to be. Cue glares from every one of her teammates and the Big O.

  • Professor Arc:
    • Jaune's family learn about his fraudulent career only after he's gotten a handle on his situation, and show up in a ham-fisted attempt to force him to quit and return home against his will. They're unaware that he's trying to deal with a criminal conspiracy from the inside, and that he's managed to do some good for several students through his actions, treating him like the "failure" he used to be. Their attempts to handle the situation winds up making things worse for Jaune, and give the students a bad impression of them and Jaune's home life.
    • Winter combines this trope with Your Approval Fills Me with Shame: She warmly praises Weiss for helping her arrest Jaune with the information provided in the questionnaire, unaware of Weiss's respect for Jaune. Winter is caught off guard when Weiss angrily tells her to GTFO.

Sherlock Holmes

  • Love Covers All Sins: Holmes is initially too caught up in the pleasure of closing the case to realize how his methods of doing so affected Watson. After thinking about how the same situation would have affected him had their positions been reversed, he realizes he just put his best friend through a horrifying and heart-wrenching afternoon and he dashes off to make amends.

A Song of Ice and Fire

  • and love is not a victory march: Edmure asks Catelyn if the Blackfish killed Ned, mostly because he doesn't understand the finality of death and the futility of war.
  • The Mountain and the Wolf: The Wolf's defining characteristic is that everyone he talks to is going to want to punch him sooner or later, because he keeps saying exactly the wrong thing to them without even knowing it.
    • Telling Theon Greyjoy to grow a pair.
    • Thinking (and saying) Jon is too squeamish to see men being executed by the same person who ordered the execution, when this was one of Ned Stark's biggest character traits/flaws.
    • Talking about burning ships in Blackwater Bay while next to Davos Seaworth (who lost a son when Tyrion set the bay on fire).
    • Claiming something reeks in the presence of Theon.

SpongeBob SquarePants

  • In Mortal Man, Patrick makes a comment about how, even if Mermaid Man is gone, they've still got Barnacle Boy. Spongebob cringes a bit at the comment's tactlessness, but he also understands that Patrick was only trying to help.

Spyro the Dragon

  • The Portal: When Alex first meets Storm, he tells her that he is actually a human turned into a dragon. Storm, quite understandably, goes into shock.
    Storm: Who are you and what are you doing?
    Alex: U-um... Alex.
    Storm: I'm Storm.
    Alex: And as for what I'm doing, I have no idea. One minute, I'm human, and the next, I'm walking on all fours covered in scales.
    (Storm exits)
    Alex: Uh oh. Maybe telling her that wasn't the best of ideas.

Star Trek

  • A Long Way Home has Cadet Kirk venting his frustrations regarding his Vulcan professor Spock to a girl he just met and finds interesting. Unfortunately for him, not only did he use a racial slur to describe Spock (who is from the same species as the girl), said girl also is Spock's younger sister. To his credit, James immediately feels like garbage when informed of that and doesn't begrudge T'Harauk for punching him.

Star Wars

  • The Desert Storm:
    • After Ben issues his open challenge for custody of Obi-Wan, Yoda asks Ben if he believes he could beat him. Ben’s casual assurance that Yoda could “win the moment [he] wanted to” actually saddens Yoda since he had been waiting to pass away from old age (as he's already well beyond his people's standard lifespan) and didn’t know that he still had a few decades left to live. Ben quickly realizes his mistake and apologizes, telling Yoda that he's got time left.
    • In Remembrance: Part II, Mace’s attempt to protect Obi-Wan from scrutiny by pressuring him into answering the Council’s questions about Ben comes across as this to Obi-Wan since it shows that the Council has already decided that Ben is dangerous and believes that Obi-Wan needs to be protected from him.
  • The Naked Jedi: Briefly applies to Sarza when talking with her new apprentice, Meelan Lah of the Yuuzhan Vong; it is considered offensive among the Yuuzhan Vong not to refer to one by their full name, but Meelan Lah forgives Sarza the initial mistake as she recognises that Sarza didn't know that would be offensive to her.

Steven Universe

  • In Faded Blue, it's not that Steven is trying to insult Pearl or Garnet. It's simply that all he knows about Gem culture he learned from Blue Pearl. And once he does learn what he's done is not correct, he does try to make up for it.

Tolkien's Legendarium

  • In The Silmarillion fanfiction A Boy, a Girl and a Dog: The Leithian Script, the main characters explain to Maiwe -an Elf- what sickness means to Humans. Shocked, Maiwe remembers out loud she heard other Elves talking about "the Sickly Ones", at which point she is warned that is a racial slur. She's awfully sorry but Beren -the Human in the group- doesn't take offense since he knows she didn't mean it.
    Beren: (to the others, earnestly) She doesn't understand. How can she? No one who stayed has ever met us. You said that not even animals get sick here the way they do back home, there's no blight on crops, things don't grow wrong, they just grow until they get old and stop, or something eats them first — they don't start dying while they're alive.
    Teler Maid: (sudden understanding) That is what they meant, those I did overhear talking that are returned, when they spoke of the Sickly Ones —
    Ranger: (fierce) You mustn't say that —
    Teler Maid: (concerned) Is that unmannerly? Was that insult, then?
    Beren: (shaking his head) Not from you, no.
  • In The Hobbit fanfic Wear a Flower in Your Hair (You're Beautiful), Thorin calls Bella a halfling in reference to her being a hobbit. Unbeknownst to Thorin, "halfling" is an insult hobbits use to describe females who are barren.

Touken Ranbu

  • Tales of the Undiscovered Swords: After Sasanoyuki returns from his highly traumatizing kiwame training, his brother Izuminokami drags him into performing tameshigiri with the intention of showing off the Kanesada's cutting power, not knowing that it's a Trauma Button reminding Sasanoyuki of his own historical cutting test from which he learns of his sharpness, leading to a long period of guilt and self-hatred in the past. Sasanoyuki ends up attempting suicide after this.

Victorious

  • "Twist of fate" opens with Tori's first day at Hollywood Arts, where Jade becomes frustrated at Tori asking her for directions and tells her to "run off and play hopscotch". Tori is offended as she is paralysed from the waist down and has to use a wheelchair, but this insult qualifies as "innocently insensitive" as Tori learns later that Jade was blinded by a stroke a few months ago and therefore legitimately didn't know that Tori was in a wheelchair.

Worm

  • Public Relations Nightmare: Lily and Taylor both manage to insult each other unintentionally in the latter's first days as a Ward. Taylor avoids Lily because her appearance both in and out of costume reminds her of Sophia, who helped cause he Trigger Event, combined with just a general avoidance of socializing due to a distaste for teenage drama. Lily, upset at Taylor's avoidance and frustrated with romantic problems elsewhere, looked up Brockton Bay where Taylor came from and came to the conclusion that she might be a former Empire 88 member or sympathizer. The end result is Lily having to go through sensitivity training for accusing someone of being a nazi and Taylor bearing a bit of a grudge against her.
