But we know that modern medical technology, including that used by the Cardassians, can change your face to anything you want it to be.   • Indications are the Prin suffered some form of mental disturbance in the blast, in the way that head trauma or stroke have been known to result in personality changes - any imbalance he might have already had is aggravated as a result, in which case, he may not have sought treatment. Alternatively, Cardassia is resource poor, particularly in comparison to Bajor prior to the Occupation or the Federation in general, which may have meant that he DIDN'T qualify for treatment - look at the modern United States health care system, where people delay or fail to have major surgeries due to an inability to cover the financial costs of the treatment. Medical treatment in the Cardassian Union may have been in a similar place where he couldn't qualify for the necessary surgery.
