It's very well possible that the brother turned Aki into the monster she is now and Aki's casual willingness to rape a girl might suggest something even more sinister about their relationship.
Unlikely, since he kept asking her to tone it down. He actually seemed intimidated by her, not controlling, and was the passive partner in their almost sex scene. Fridge Logic
At the end we learn that Shiina's dragon really is Earth. Now consider all the mining of natural resources and other stuff humanity is doing to Earth. Combine it with how all punishment inflicted on a dragon is telepathically relayed on its user. Shiina should be suffering constant excruciating pain or at least severe itching all over her body.
Shiina only connects to her dragon at the end of the series and she immediately wipes out all life on earth, she probably felt it then.
