: Sirius complains about Remus referring to Severus Snape as "Severus", when Snape doesn't respond in kind, always referring to Remus by his last name. Remus just grins and points out that it's not actually a courtesy.Remus: Ah, yes, but he loathes it when I call him Severus so I do it at this point to merely annoy him further. - Hetalia: Axis Powers fanfic Gankona, Unnachgiebig, Unità: Germany, Italy, and Japan used to be on country-name basis. However, Italy momentarily switched to first-name basis to make his words more impactful to Germany and Japan, and they exclaimed his first name in shock. However, over time, they permanently switched to first-name basis.
- In Kyon: Big Damn Hero, members of the SOS Brigade change the way they address each other as a symbol of their True Companions relationship is strengthening: Tsuruya is called Tsu-chan by Haruhi and Mikuru(big), and Yuki and Mikuru being called by their first names not only by Haruhi.
- Used in the The X-Files/Earth: Final Conflict crossover "Apologia". The young Agent Sandoval asks if he can address Scully as "Dana", because he refuses to give the impression that he's trying to take Mulder's place.
- A classic example of this trope appears in With Strings Attached when, as a sign of his complete Heel–Face Turn, the Hunter asks Paul to start calling him by his first name, Jim. He had already agreed to use the first names of the four that morning, rather than the insulting nicknames he'd come up with for them at the beginning. (Originally, when George said that they have names and would he care to hear them, he said "Not especially.")
- In the Katawa Shoujo fic From Shizune's Perspective, Shizune temporarily switches to calling Emi by her first name rather than "Miss Ibarazaki" to make a point, then despite switching back to calling her by her last name, gradually switches over to her first name. She also points out that Hisao might be "interested" in Emi if he calls her by her first name, causing Misha to react in displeasure because she is in love with Hisao. Shizune also calls Misha by her first name, Shiina, in this fic, instead of her nickname.
- In Ace Combat: The Equestrian War, Lightning Bolt, Twilight Sparkle and Tornado Swirl are usually called Lightning, Twilight and Tornado. Cloud Kicker also applies to this, though rarely.
- Jessica Poole gets onto this with Apollo Justice: Ace Attorney during the second day of investigation in Apollo Justice: Ace Attorney Case 5: Turnabout Substitution, but retracts it the next day, much to Apollo's surprise.
- In A Complete Turnabout, one of the first clues that Phoenix gets regarding his new place in an alternate universe is that Edgeworth calls him "Phoenix" instead of "Wright," like canon!Edgeworth always does.
- A major part of Vinyl Scratch's Character Development in Vinyl and Octavia Machete Their Way Through The Jungle is the fact that she stops calling Octavia names like Tavi and Octy and instead calls her by her full name.
- Calvin and Hobbes III: Double Trouble has two characters named Rupert Chill - one human, one alien. The alien Rupert goes by his first name to avert confusion.
- Nav from Diaries of a Madman tends to be on first name terms with almost everyone he meets, royalty and commoners alike. Strangely enough, it's usually those accompanying him that protest over the lack of formality, rather than the recipients.
- Yuzu and Byakuya ran the whole gamut of names and titles in the Bleach fanfics Phases of the Moon and Chasing the Moon. While Yuzu was in the Academy and Byakuya was helping her train, Yuzu called him "Kuchiki-dono" (Lord Kuchiki) and Byakuya called her "Kurosaki Yuzu" (if he called her anything at all). While Yuzu was still an Academy student but chose to actively involve herself in a battle for the first time, she called him "Kuchiki-taicho" (Captain Kuchiki), signifying that she now sees herself as a shinigami and a (future) member of the Gotei 13. Ten years later, by the time of the sequel, Byakuya now calls her "Kurosaki", seeing as she's now his vice-captain, and Yuzu starts referring to him almost exclusively as "Taicho" (just plain Captain). As the Will They or Won't They? intensifies, they start thinking each other's first names without actually saying them out loud. They finally make it to First-Name Basis at the end of ''Chasing The Moon's'' penultimate arc. Too bad it happens in the last moment before Yuzu runs off on a Suicide Mission that appears to be Unwinnable by Design.
- Discussed in "Aen'rhien Vailiuri". Tovan tr'Khev tells Jaleh Khoroushi to call him by his first name, because Morgan t'Thavrau, his neighbor and friend for ten years on Virinat and now his captain, doesn't do that anymore and he misses it.
- In Out of the Dead Land, Bucky initially switches back and forth between referring to himself and others in his head by their codenames or by their first names depending on whether he's trying to be the Winter Soldier or Bucky Barnes at that particular moment. As the fic progresses and he begins regaining his sense of identity, he eventually transitions to using first names only. Significantly, he does this first with Steve.
- In Shatterheart, Kurogane usually refers to Real!Syaoran as "kid;" but when he and Real! transition from friends to Friends with Benefits, Kurogane makes a point of calling Syaoran by his name when they're alone. Kurogane temporarily rescinds this when he ends their relationship. He goes back to using it when he and Syaoran rekindle their bond.
- In Vapors the main character muses on how few people ever even talk to a kage, let alone have a First-Name Basis with three of them. Later on, Anbu captain Fish has been grumpy that he got snubbed by being given a rookie agent for his team. Then they report to the Hokage, who immediately greets Butterfly by name and personally heals her rather minor injuries, while Fish silently freaks out.
- Fire Emblem Fates: Aftermath: Due to their close friendship, Silas is not obligated to call Corrin by his title of "King" and can just refer to him by his first name in casual conversation.
- In Return of the Hero, upon learning that Kelly isn't his biological mother, Nate starts addressing her by her name instead of "Mom". He reverts back to calling her "Mom" when she is taken hostage by Cassidy and Butch.
- In My Little Pony: Friendship Is Magic fanfic The Terrifying Prospect of Swans in Love, Twilight invariably calls Princess Celestia by her title. However, when she's angry, she slips and just calls her Celestia. At the end of the story, after they get together, Princess Celestia says she would simply like to be called "Celestia" by her henceforth. Twilight jokes that Celestia is ruining all her teenaged fantasies, but likes it better this way.
- In All This Sh*t is Twice as Weird, Victoria urges the newest member of the Inquisition to "Call me Victoria. Not enough people around here use my first name." (She's normally addressed as "Lady Inquisitor" or "Your Worship.")
- In the Kim Possible fic The Touch of Green Fire, Team Go refer to each other by their superhero names. Shego is the exception. She doesn't like using the names and instead calls her brothers by their actual names. This also shows that she is the odd one out who doesn't want to be on the team.
- In My Huntsman Academia, greeting and social conventions follow the western-style of RWBY, so Izuku gets referred to by his first name a lot more than in his home series, as do Tenya, Tsuyu, Shouto, and Katsuki. Only their teachers, Tenya, and Weiss refer to them by their surnames, reflecting their more distant and professional relationship. Weiss quickly loosens up enough to refer to Izuku and the others by their given names while Tenya remains uptight until others others remind him that he doesn't have to be so formal.
- In Karma in Retrograde, Touya insists that people refer to him by his given name rather than his surname, Todoroki. This is both to distance himself from his father and to distinguish himself from his younger brother, Shouto, who is used to being referred to as Todoroki.
- Where Talent Goes to Die:
- Ami Yuuki refers to all her classmates highly informally, using their first names and "-kun" for boys or "-chan" for girls.
- The use of first names is shown as proof of Kaori Miura's evolving relationship with Sae Edogawa. Shortly after meeting Edogawa, Miura calls her "Sensei" as a form of respect for her favorite author. At the end of Chapter II, Miura, having resolve to treat Edogawa as more of an equal, starts calling her "Edogawa-san." Early on in Chapter IV, Miura and Edogawa switch to calling each other "Sae" and "Kaori" without honorifics, signifying how close they've become.
- Somewhat similarly, Kuro Akasaka and Komaki Katsura start calling the others by their first names and the "-chan" honorific in Chapter III.
- Where Talent Goes on Vacation:
- Tatsuki and Taiga Tachibana, who are twin sisters, suggest that their classmates call them by their first names to make it clear who they're talking to.
- Himeno Himemiya and Kanae Tsukimura, the latter of whom is the former's maid, also use first names on each other, despite using last names and "-san" on most of their classmates. Tsukimura calls Himemiya "Himeno-sama"(although Himemiya wishes Tsukimura wouldn't use "-sama" on her) while Himemiya calls Tsukimura "Kanae".
- Sayuri Sasaki asks permission to use first names with those she meets, but respects the wishes of anyone who says no. Tsukimura soon starts using Sasaki's first name.
- In Despair's Last Resort, while first name basis is generally the norm (except for Monokuma, Chiyo, Arata and Kazumi), there are a few cases in which the switch to using first names can be significant. While the ultra-polite Chiyo usually calls her classmates by their last names and uses "Mr." or "Ms." as appropriate, she calls her Childhood Friend Kumiko by her first name. Kazumi starts calling her best friend Shizuka by her first name shortly before the latter is executed, and Arata calls Takara by her first name as a show of respect after she manages to convict him for murder.
- In Roadside Assistance, Yang disapproves of Weiss (or, apparently, most people) calling her "Ms. Xiao Long". Several minutes later, Weiss finds herself acquainted with Yang enough that she tells her not to address her as "Ms. Schnee".
- Team Rocket Roots: Not even James' best friends and teammates know his surname, which works to his advantage as no one connects Morgan Minerals with him.
- After Ping in Make a Man Out of You tells Wei that he views him like a brother, Wei requests being called by his first name, instead of "Zhang". Ping agrees and tells Wei to call him by his first name too.
- Used variously in The Mountain and the Wolf. While the Wolf is on a first-name basis with his crew, he refers to those he's apparently respectful towards by family name (such as Jon as Snow or Davos as Seaworth) or some distinguishing characteristic (Daenerys as Dragonqueen, Grey Worm as the Worm, Jaime as gold-hand, Tyrion as Shield-slayer), their job (fleshcrafter/necromancer for Qyburn) or by an insult for people he doesn't like/is trying to kill (Molehill for the Mountain, Snolton for Ramsay, Fingers for Baelish, rust-born for Euron, whore-queen for Cersei). He does this even when talking about them to another person.
- By the Sea: After Obi-Wan has his freakout about Cody turning out to be royalty, he gathers himself up and asks Cody if he should be using a title instead. Cody, bemused, says no. Of course Cody would find that weird; he considers Obi-Wan his lover at that point, and who calls their lover a title to their face?
- Tales of the Canterlot Deportation Agency: Jack: "All In", a former bully refers to a classmate by her surname, Quinzel, and as a sign of how she's willing to be more friendly to him, she responds with her first name, Harleen.
- In What Could Have Been, Nie Mingjue uses Jin Guangyao’s full name for the first time within his earshot after seeing him with his nieces and nephews.
