With the success of other Hellraiser comics, Epic Comics began another spin off for the Hellraiser franchise. This series ran from 1989 to 1993, with creator Clive Barker acting as a consultant on all 20 issues. This series has at least one Spinoff, Clive Barker's The Harrowers.

- Hellhole Prison: La Enfermidad, from Issue #2's "The Vault", is a "strictly-modeled version of Hell, created on Earth, by one man".
- Politically Incorrect Villain: Harmon Taylor, from Issue #13's "The Ferryman", is a high-ranking member of the Ku Klux Klan who kidnap African Americans and takes them back to his "floating house of horrors", where men were tortured and forced to participate in Gladiator Games, and the women were raped before they were thrown overboard directly into deep water, abandoned to drown. Taylor took some of his victims to a deserted island and set them free with the sole intention of Hunting the Most Dangerous Game.
- Wardens Are Evil: Arturo Velez, from Issue #2's "The Vault" is the "Jefe of La Enfermidad".
