  • The opening theme, "il" note , is a snappy jazz piece that effectively sets the tone for the visual novel and captures the setting of the colorful and chaotic city of Shinkoumi.

Taku's Ending Themes (Artist: THE ANDS)

  • "inside out" is both somber and haunting, with its lyrics and tune perfectly describing one's slow decline into insanity, loneliness and despair.
  • "arlequin" is a slow and calm shoegaze piece that captures the feel of how, in spite of going through so much pain and suffering, things will eventually turn out okay as long as one holds on.

Rei's Ending Themes (Artist: GOATBED)

  • "HEAD CASE TATTOO" is a good blend of bass and electrical music, resulting in a slow, dark yet groovy beat that leaves a sense of disquiet.
  • "Genzai kara Mirai e" note  is a lively and energetic piece that shows that the same band who brought you several opening and ending themes from DRAMAtical Murder is still at the top of their game with producing techno pieces that are easy to jive to.

Madarame's Ending Themes (Artist: ARKTA)

  • "Damaging you" is reminiscent of Linkin Park's music and embodies the concept of destructive catharsis with its lyrics about inflicting pain and torture on both someone else and on one's self.
  • "Keep me alive" is utterly raw in tune, vocals and lyrics, and encapsulates the essence of both defiance towards meaninglessness and loyalty to one's desires.

Fujieda's Ending Themes (Artist: Anna Evans golden folks note , KiRi note , THE ANDS note )

  • "Shinjirou" note  is a funky and bewitching theme that compels anyone listening to it to sway their body to its beat. The fact that this is a bad ending theme doesn't deter fans from regarding it as an alluring and catchy song.
  • "lullaby" is a calm, ethereal, and heartfelt piece that will move all to tears with its melancholic but nevertheless beautiful melody.
  • "after all", the last ending theme of the game, is both lively in tune yet calming to the heart, letting the listener feel in full how Towa has achieved his happy ending and that his long journey of pain has finally ended to make way for a new and better one.

    BGM Tracks 
  • "Danger" (Track 14) lives up to its title, with its intense and heavy beats bound to make the listener's hairs stand on edge at the moments of tension that occur in the visual novel.
  • "Deathmatch" (Track 15) plays in almost all fight scenes, and it captures the feeling of adrenaline rushing in one's body as they get into the heat of battle and get pumped up with the thrill of fighting.
  • "Teardrops" (Track 19) is slow and gentle piano piece that plays during the more solemn moments of the visual novel. What is notable is that this track will always play in the Madness endings as well, showcasing that while the bad endings in Slow Damage are undoubtedly horrifying, they are ultimately tragic.
  • "Resolve" (Track 47) seamlessly combines the organ, the electric guitar, and a One-Woman Wail into a powerful track that plays only in the final confrontation in Fujieda's route, as if to say that by then, all the pieces have come together, all that's left are the last players standing to settle things once and for all.
  • "Maya" (Track 48) is both solemn and hauntingly enchanting, and is similar to a siren's song—much like how Maya herself is a siren, glamorous and alluring but ultimately deadly and merciless.
  • "Truth" (Track 49) is a track that only plays at the scene where Towa finds his mother's diary, and gives the player the feeling that Towa's search for his past is really about to come to a close, with the closure he needs leaving him more emotional than he would ever expect.
  • "Day to Day" (Track 52) is a Triumphant Reprise of both "Joy" and "Determination" that plays in the very last scene of the Golden Ending. It encapsulates the emotion brought forth from the scene where after finding a painting from his childhood, Towa is overwhelmed and with all kinds of feelings, and reflects on his newfound resolve to live and to cherish his life.
