Kisegach is extremely cold to Bykov after him cheating on her. She even refused to pay him his bonus money. Meanwhile, Phil learns about a labor codex violations, and decides to organise a strike.

Varya must once again work under Kupitman... and she's ready to do anything, up to and including doing his chores, to avoid actual venereology-related work -- because she is deeply afraid of it. Kupitman considers this to be an anomaly, and any medical anomaly must be treated. Unfortunately, the problem becomes worse when Varya notices some acne, and decides that she is now infected... and attempts to "cure" it with various drugs are only making the situation worse.

Bykov learns about Phil's strike attempt, and decides to use it in his favour. But once he persuade Kisegach into paying him his money, he immediately betrays Phil and forces the strike to end (by lying that he was fired, and same fate awaits anyone who would try, which forces everyone to abandon Phil). Bykov also mentions what according to that same "labor codex", he can simply fire Phil for that, but instead would just punish him with night shift.
* ATasteOfTheirOwnMedicine: After every other revenge idea against their CrankyNeighbor proves to be too insane (or criminal) to work, Gleb and Semyon decided to just imitate her own behaviour. But before they managed to proceed with it, they realised what she was already discharged from the hospital.

Fortunately, she is not contagious.
** Later Varya noticed some acne, and decided what she was infected by that girl anyway. Not even Kupitman managed to convince her otherwise, [[SanitySlippage and situation quickly escalates to near insanity]]. She also take several drugs in attempt to "cure" herself, which predictably made situation worse. Kupitman only snap her out of it when he humiliate her before other patients, and shame overwhelmed fear. Unfortunately, now she must treat all those patients as punishment.
* ThisIsGonnaSuck: {{Invoked}} by Gleb, once he realised what their new patient is their [[CrankyNeighbor neighbour]], and tell her so; for Gleb himself, it is [[InvertedTrope inverted]], because this is perfect opportunity for revenge. Only problem is to decide ''which'' method to chose.
* WomanScorned: Kysegach still can't forgive Bykov for his infidelity, and decided to use her [[ObstructiveBureaucrat position as Chief Doctor to make his life living hell]].
