In this case, his condition is not a mystery--he's clearly dying from Acute Radiation Syndrome, with his burned skin decomposing on his body. However, the nosebleed is another symptom of the horrific bodily breakdown mentioned early by Legasov: his blood vessels are beginning to disintegrate, which means he can't even be given medication for the pain anymore.
