** Actually, Pyramid Head was just the guilt. The nurses and [[Maria]] were frustrated sexuality. Also, there's no indication that Colin ''became'' Pyramid Head.

** By the time you witness the burning of [[spoiler: Officer Bennett]], you just couldn't want her to die any more than you already do.

* DistaffCounterpart: Rose to Harry; the director changed the main character's gender because he felt like the main character of the video game was very feminine already because he [[UnfortunateImplications cared about his daughter enough to look for her]]. Also Harry, the lead character in the game screamed and fainted too much, and being male clashed with their film's theme of motherhood.

** Watching Cybil get burned to death was plain [[HighOctaneNightmareFuel gruesome]], but seeing Cybil's smoking corpse in the background afterwards was a moment of pure narmage.

* TrailersAlwaysLie: In one part of the previews for the movie it showed Alessa saying in a demonic voice "I am the reaper." This is not in the movie.
** Well, she does say it, just in her regular speaking voice.
* UnfortunateImplications: Woo, ''boy''. The most glaring is the declaration of motherhood as essential while completely condemning fathers, as the only father in the movie is Chris, who doesn't care ''at all'' for his adopted daughter (he never mentions her to Officer Gucci, for example), while Dahlia, who ''allowed her daughter to be burned to death by a cult'' is shown to not be bad after all, and earns a RedemptionEarnsLife ending. Alessa's father also is never mentioned. Also, the reason behind the decision to make the lead female; because the leading guy in the game ''acted too feminine''.Oh, so now caring for your kid is unmanly? That's probably the most Chauvinist and offensive thing I've heard in my life. And that was the reasoning of the director, [[FrenchsAreJerks who's French]].

** To call the cult in this movie "Christian" is definitely doing a ''massive'' disservice to every Christian on the planet.

* RealPlaceBackground: Some of the outdoors scenes were shot in the downtown section of Brantford, Ontario. A local joke is that all the set dressers had to do was add garbage cans and ask everyone to leave for a few days.

** Although this is arguable, given the cult's specific, not-particularly-Christian symbol, and the fact that puritanical Christians don't have a copyright on calling people witches or using the word "sin".
*** It's pretty much implied that they used to be a Christian sect, but their obsession with witches went so far that nearly all the other trappings of the religion were marginalized, and few Christians would consider sharing the same religion as these people - even serious fundamentalists.

* AndIMustScream: Part of [[spoiler: Alessa's]] fate. [[spoiler:Being confined to a hospital bed and covered with third degree burns for three decades isn't too good for her sanity]].
* BreakTheCutie: Alessa starts off as a pretty nice kid. Of course [[spoiler: that all goes out the window when she is abused by her classmates, a janitor, probably her mother, and [[BurnTheWitch definitely her aunt]].]] After that, you can hardly blame her for [[spoiler: literally putting them through Hell.]]
* BurnTheWitch: The town's local cult is determined to root out evil by [[KillItWithFire killing it with fire]].

* OneWingedAngel - [[spoiler: Alessa]]'s final form in the cult's church.

* WalkingWasteland: Not walking, so much, but after Alessa is [[spoiler: burned alive]], everyone and everything that comes near her is apparently killed or mutilated. The flowers left by her bedside wither and die, and a nurse who looks into her screen is immediately [[spoiler: blinded]].
** [[spoiler: I don't think the blinding would be as much that as Alessa lashing out at the nurse for looking at her with the fear she did...like she was a monster. Dunno about you, but in that scenario, when you're hurt, sad and angry, when the very person who you expect to care for you looks at you like...that, I would imagine that would kind of be the icing on the cake in all this.]]

** In a much more literal example, Colin the Janitor spreads tendrils of diseased flesh spreading over almost everything he touches- the original source of the carnivorous cockroaches, apparently.

* HoYay: Rose and Cybil have their own fanbase because of this. Made even more hilarious by the Bubble headed nurses being a sign of repressed sexuality.
