The title of this page is 'Super Mario Bros.: The Lost Levels'. The problem with this is that the '.:' looks really bad to the eye, and makes it seem like some kind of typographical error. Should it be changed?

No, we should stick to the official name of the game.

In order to get to Worlds A - D, is warping allowed or do you have to beat the game eight times in a row without skipping a stage?

You can warp all you want for those levels, warping only stops you from playing World 9 (in that playthrough). You also don't have to beat the game eight times in a row, just eight times overall.

Regarding getting to World 9 in the original, the technical requirement was actually only that you beat all 8 castles, despite what the instruction booklet said. Even if you used warp zones, as long as you beat all 8 castles, you would still get to World 9.

Try it. :)
