In The X of Y, 124 is not 12 squared. That's 144. So is this the 144th episode with X of Y or did someone just get the math wrong?

Just to clarify, this episode does NOT confirm that the John Hurt is the 9th Doctor. It merely confirms that he's used up 12 regenerations, including War Doctor's regeneration and that time 10 regenerated and kept his face.
Also according to Moffat you can use whatever numbering system you want.

Why did 11 change clothes before regenerating? When Clara enters the TARDIS she finds his old clothes laying on the floor and then he comes up wearing new ones. Doesn't make much sense.

Regarding Contrived Coincidence: Didn't the woman ask both of them both for names, Clara answered "Clara" and the Doctor answering "I'm the Doctor"? I mean "his name is the Doctor" is a significant part of the climax.
