How about the area where you find the last Chronicle of Arkham? To get a Riddler Trophy, you have to go up some stairs and override the lock on a cell door. In the cell next to that one is a one-armed patient, gibbering and staring at you. He doesn't actually do anything, but holy shit is it ever unnerving.
