One of my favorite stories of film trivia pertains to the Hammer Films version of The Mummy. The scene called for the mummy, played by Christopher Lee, to knock a door off its hinges to enter a room and attack Peter Cushing's character. The door in question was on its hinges, minus the bolts so that it would just fly off the hinges when Lee hit it. That was the idea. But a grip inadvertently put bolts in place, thinking "That door's gonna fall off its hinges." Lee did knock the door down, but dislocated his shoulder in the process. The shot remains in the film.

There was an episode of Disney's Doug where Dr. Rubbersuit creates an evil clone of Quailman, and somehow ends up with two holes in the roof of his lab. "At least use the same hole, whydon'tcha!" I was about to post it, but I can't remember where the first hole would have come from. It couldn't have been Evil Quailman, since he was created IN the lab... anyone remember?
