should there even be the films that include real world organizations? I assume this was meant for just fictional versions, otherwise you may as well include every single movie that has the FBI, KGB or SS

There seems to be a lot of crossover between this, Secret Police, and Praetorian Guard. Could the three either be better distinguished and accordingly tidied up, or just have this divided up between the two?

I think they are pretty well distinguished as is. Secret Police are merely spies or an FBI equivalent that doesn't care about civil rights. The Praetorian Guard is merely an elite unit of bodyguards. A State Sec, however, is something much bigger than either one. A 'state within a state' if you will. The Secret Police are likely to be part of, or subordinate to, State Sec but on their own they lack the military power, industrial resources, foreign intelligence services, etc... that a true State Sec has. And unlike the Praetorian Guard, which is small elite unit geared towards protecting important people, a State Sec army is just that, an army. One capable of fighting the country's regular army and winning in the event of a civil war or capable of putting down rebellions and uprisings and not just making sure The Emperor doesn't get a knife in the gut.
