Didn't the picture used to be of Neil Patrick Harris as Dr. Horrible right before he launches into "Brand New Day"? Because that one can't be topped for Psychotic Smirk.

How come this trope is named "Psychotic Smirk" when "psychotic" means "impaired by thoughts and emotions being disconnected from external reality?" The vast majority of people suffering from psychosis are dangerous only to themselves, if anyone at all.
If it's supposed to be villainous, shouldn't it be "Psychopathic Smirk"? Psychopathic implies a lack of empathy and often describes cruel or otherwise villainous people.
