I think the difference here is that it's supposed to be honest and legitimate, and something that the character had really wanted to say for awhile, but what's a big deal to them doesn't even register to the person they're confessing to.

There's an awful lot of overlap here with Sarcastic Confession — Grosse Pointe Blank, American Psycho, Breaking Bad, none of those guys would have said what they did if they thought it would be taken seriously.
