A correction:

"Maro was submitted to the development file for Mirage with its creator's username (Ma Ro for Mark Rosewater) for the database instead of a placeholder name. No-one noticed the mistake until the Creative department had already commissioned the art, so it stayed."

Maro's at was never commissioned. The art director at the time saw the art (entitled "The Green Man") and asked the artist for the rights to it. SAUCE!:

Wouldn't Asagi from Disgaea count? I mean, her entire character is based off a game that was canceled, leading her to try to take over other games by force to become the main character.
