Penny Arcade did one where against Jim (now a skeleton), Tycho pulls out his red d20 and says "Roll for initiative." He rolls a 20... and Jim goes "Good Roll" and gets the others on his side to pack it in.

In order to be our definition of a, critical hit does there have to be an element of randomness. In tf2 the average weapon is random, but there are other weapons that can get crits like, frontier justice and the diamond back if certain, non-random things happen.
I'm afraid I speak from experience on the dental thing. Sweet mother of crap, that hurt. But I hasten to add that it was over fast, and I've only had it happen once. (Wasn't my dentist's fault either; he can't exactly see through gums.)
