Is there a variant of this trope that can apply to characters In-Universe rather than the actors portraying them? Dr. Van Helsing's "King Laugh" scene from Dracula instantly comes to mind.

What on Earth does, "Respiration nil, Aston Villa two" mean?

That's how football scores are reported — (home side) (goals scored by home side), (away side) (goals scored by away side). Miller's actual line was "respiration nil", i.e. "patient is not breathing." He whispered the rest into Sladen's (i.e. the not-breathing patient's) ear, and hilarity ensued.
