Duke Nukem 3D did that medkit landing on purpose just to tell the player that a) there is fall damage and b) healed what would be inevitable damage, out of mercy, right at the start of the game. Just stating the obvious, good game design.
There should be a new trope just out of it, something like "Show by doing", because Half-Life took that trope and ran with it, all over the place. The ride on the tram simply lets you learn to walk around without falling off it, the gravity gun, the crowbar, the first pistol, every single first interaction of Gordon with any object is shown or lets you free to experiment it, or it is mentioned right before he can interact with it himself.
