I don't understand Mamoru Chiba's death being uncertain in Sailor Stars. We last see him on a plane, the plane is hit by a ball of energy that envelopes the entire plane, then cut to the Starlights saying "Another star has gone out" which is about as explicit as you can make it that the man is dead. Sure we don't know HOW he died until the end but it's pretty certain he's gone.

Is South Park's Chief an aversion?

Does the Wrath of Khan example qualify? Her death became kind of a minor plot point since it was on Khan's laundry list of grievances toward Kirk.
