Was it explicitly said that the Lyctor of the Second is named Gideon and not Ortus ? Or is it just readers putting two and two together about the note from the first book and Harrow's rewriting of her memories?

It wasn't explicitly stated, but it's more than just putting two and two together from the note and Harrow's memory rewriting — Harrow has the same physical reaction (i.e. passing out) to the Lyctor's name being said as she does to Gideon's name being said and replaced in her mind with Ortus, so it's a pretty clear parallel.
