The first headscratcher (re: Meat Puzzle) actually isn't one. Local authorities had no idea the victims were murdered, they only knew they disappeared. And given that the trial took place several years in the past, there was no reason to connect them over it. Ducky only made the connection after he identified the DA.

And Ducky actually explains why NCIS is working the case: The barrels with the bodies were dropped on the premises of the Bethesda Naval Hospital (now "Walter Reed National Military Medical Center"), which puts them into NCIS jurisdiction.
