So, in a way to understand how this continuity works, should we assume the Optimus Prime from War / Fall Of Cybertron is the same version as the Optimus Prime from the Transformers Prime cartoon on The Hub?

Yes and no, you should assume Prime!Optimus's past closely resembles a combination of WFC and Exiles but none of them fit together perfectly though
