  • Alternative Character Interpretation: Regarding Dr. Koji Kuroe: is Professor Tsuchida correct in his allegations that he cared more about not risking disbarment than saving Shizumi's life? Or was Tsuchida refusing to accept that Kuroe didn't have the skills necessary to save her? In the Flashback, Kuroe protests that attempting to extract the bullet is too risky, and balks at Tsuchida demanding that he steal supplies from a nearby ambulance in order to make the attempt. The fact that Khufu does not hold it against Kuroe suggests the latter.
    • Related to the above, just how much — if at all — does Kuroe regret that incident? Does he regard it as his greatest failure, or something less so?
    • Another source of this is the actions of Pharaoh Khufu. It's suggested that their situation was a Fate Worse than Death, making their defeat at Ayuto's hands a Mercy Kill... But Kyosuke could also just be saying that to try and comfort Ayuto and snap them out of their BSOD. And did he pass his powers on to Kyosuke because he believed he was truly worthy, or just to be rid of them, deeming Kyosuke the best candidate of the lot by virtue of elimination?
  • Complete Monster: Soji Mizumi is a vile Serial Rapist using his photography career as a cover to find material to blackmail others. One of the people tricked into the pyramid by Professor Tetsuya Tsuchida, Mizumi hides who he truly is until he finds drugs on Miss Otogi, which he uses as blackmail material to rape her while bragging about having done this to others before. After attempting to rape a monster he thought was a woman, if he survives he seemingly helps in the battle against Khufu, only to run, leaving the others to die.
  • Cry for the Devil: While Professor Tsuchida is a complete Jerkass for most of the game, his motivation and final moments may evoke some sympathy, as it's revealed that his daughter was killed in a terrorist attack, bleeding out before his eyes. While Kuroe was present, he didn't have his medical license and believed that he couldn't help; Tsuchida interpreted this as him putting his career ahead of saving Shizumi's life.
    • To help twist the knife, if little Rin is still alive, she pleads with him to hang on, calling him Grandfather. Hearing this appears to help trigger a Heel Realization... or, if nothing else, gets him to muse about how he never expected to be called that.
  • Fridge Horror:
    • If you fail to save characters, they will all come back as reanimated enemies that attempt to kill you. However, given the context of Kyosuke gaining Khufu's powers, which is before the Escape Sequence, it brings up an interesting question: Were all the killed characters coming back as horrifying undead a product of the ruins, or was it induced by a combination of Kyosuke's new powers combined with his Survivor Guilt?
    • If he makes it out with his life, there is no indication that Soji Mizumi's true nature has been publicly exposed, and he won't face mortal justice for being a Serial Rapist. How many more women will be victimized by him?
  • Nightmare Fuel: The various judgments, but Rin Tsukihara's stands out for the sprite work involved — fail to save them, and Ayuto's Forced to Watch them spasm where they're pinned against the wall, eyes bulging and rolling up into their head as they bleed out.
    • The escape sequence, depending on how you played up to that point. Everyone who died (except Tsuchida and Kuroe) comes back as a reanimated zombie. What's worse, the game can pull a Hope Spot and make you think that they actually survived, before pulling the rug out from under you.
