  • Funny Moments:
    • Upon taking their first few steps into the abandoned West Nira Hospital, Takeshi immediately passes out onto the floor.
    Ryan: He fell down!
    • When Cassey asks Hiroshi what happened when he was with Ryan, the player has a dialogue option to make him sound like a chivalrous knight who valiantly fought off the oni by himself, adding that he jumped in the way to save Ryan. Cassey doesn't buy it, however what she says in response really sells it.
    Cassey: I might have believed you if you didn't add that last part.
    Hiroshi: Well, it was worth a shot.
  • Nightmare Fuel: Being a fangame of Ao Oni, it's quite reasonable for there to be a lot of disturbing imagery.
    • The fact that it takes place in a poorly lit, abandoned hospital building instead of the original, well-lit mansion should say something about how effective the game's environment is.
    • The main Oni in this game is a lot more freaky looking while still retaining the traditional unsettling look of the original Oni. This one is a pale white with one eye stitched shut, and is constantly baring it's large, gnashing teeth. Best hope it doesn't find you while hiding in one of the closets...
    • Other than the main one, the other notable Oni in this game is the "Blob Spider Oni." This is a monstrously sized Oni that has several skittering legs and an enormous blinking eye in the center of its gelatinous abdomen. Even worse, it's impossible to outrun this Oni and you're instead forced to locate a sturdy piece of furniture to barricade yourself with in order to shut off the infinite chase timer.
    • The secret area you can unlock when you collect all the voodoo dolls has a few noteworthy elements that make it spooky as all get out.
      • First of all, the ambient music that plays in the area is nothing but deranged, off-key stings and hums with intermittent snapping noises mixed in. There's no melody present at all and is overall just plain unnerving to hear.
      • Then you come across a room that has the corpse of a few roach onis and a giant spider oni. It's bad enough that you have these pale white demons chasing after you, but could there have been something else, something even more deadly than the onis, lurking in this building that the gang never ran into?
      • Then finally, you have the final corridors that lead to the remaining keys needed to escape. These rooms are twisted and unnaturally distorted while a static filter makes them stutter erratically. Additionally, this distortion effect is not seen anywhere else in the main route, so it's not something the players would be accustomed to.
