  • Complete Monster: The Witch was a human woman who became the Queen of the Underworld upon her passing because of her sheer, unfettered evil. The Witch is the mother of the Player Character, as well as one of the villagers, Amy Cooper, each of whom she sired in a complex gambit to damn the entire town of Lakeview Valley to eternal torture. In the endings where she succeeds in corrupting her offspring, the Witch has the townspeople massacred and their souls brought to the Underworld, where she leaves them to the ravages of the demonic fiends; what follows is an orgy of rape, torture, forced incest, and Body Horror as the fiends torture their victims in every wicked way imaginable. Entire families are Forced to Watch each other being tortured, their eyelids peeled away, and victims are mutilated beyond the point of any recognition. The Witch saves the worst fate for her own daughter, Amy Cooper; disgusted with Amy's failure to live up to her evil potential, the Witch has her gang-raped by the fiends past the point of the Despair Event Horizon. Even her care for the player character is dependent entirely on their usefulness to her; in the endings where they die, the Witch gloatingly rules them a failure, telling their murderer "you can kill as many of my children as you want".
  • Crosses the Line Twice:
    • Among all the horror of the Lake Day massacre, Old Ruth stands there grinning and giving a thumbs up.
    • Morgo's "punch a clown, he won't frown" act, where he used to punch Bum Bum in the face on stage. The idea is disturbing, but with the catchy tagline it's also kind of funny.
  • Nightmare Fuel:
    • The close up of Amy's body after being hit by the bus is quite disturbing to look at. And if you talk to her, she screams "The blood flies! The blood flies!" on repeat.
    • While wandering around at night you can come across the Dweller, a shadowy monster which stands in the darkness and quivers. Talk to it and it will say "Mirror, mirror... On the wall... Who will eat the hearts of them all?". Attacking the creature with any weapon other than the Sheriff's sabre causes it to instantly kill you. Defeating the dweller isn't any better, as this causes it to hide under your bed and whisper things while you sleep.
    • The scarecrow in your garden looks extremely uncanny and somewhat like a stuffed corpse. At night it gets up and wanders around, searching for an old doll. Finding the doll will provoke it to attack you, and it will turn to a centipede-like creature before giving you an eyeless jumpscare.
      • Kill Old Ruth, and the scarecrow will stand outside your front door the next day and speak her name. Later, the scarecrow will be replaced with Ruth's severed head on a pole.
    • The Lake Day massacre, where you see all the people you've gotten to know and care about ripped apart. Morgo really does not mess around, the brutality and efficiency that he kills people with is frightening.
      • If you agree to help Morgo, he will arrange the victims' mutilated bodies into a deranged work of art resembling a giant sigil or eye.
  • Nightmare Retardant: A creepy pair of glowing eyes follows you around the town to whisper sinister things into your ear...and also give you an Info Dump about saunas.
