• He Really Can Act: Most fans and critics agree that musician Corey Taylor and adult film actress Angelina Armani perform pretty good roles as an irritable orderly and The Woobie, respectively.
• They Wasted a Perfectly Good Character: Dylan can feel a bit underused, given how he doesn't have a phobia of his own and some major parts of his troubled yet loving relationship with Caylee take place offscreen.
• Took the Bad Film Seriously: Many people think that the actors do what they can with a cliched and relatively muddled script and make their characters easy to care about, particularly Robert Englund and Fiona Dourif as the two leads.
• The Woobie: All of the clinic's patients barely survived a horrific mass-shooting and have been plagued by horrible nightmares and supernatural forces ever since.
  • Caylee is particularly broken, as her little brother died in the shooting, she has some suicidal tendencies, and her boyfriend dies at the end of the film.
  • Dr. Andover himself is a tragic figure due to how his patients are continuing to come to harm despite his best efforts, and he is also menaced by the fear creature.
