• Jerkass Woobie: Richard is an unscrupulous surgeon with heavy ties to New York's criminal underworld, using his connections to get his bully killed, selling perscription blanks and doctors' writing samples to dealers on the street, and carrying heroin across international borders for drug runners. Despite this, he grew up in the slums with an abusive alcoholic father who cared more about getting drunk on wine than his family and died relatively young. Richard also ends up stranded on a desert island and slowly goes insane from hunger and isolation, being forced to start eating his legs to stay alive.
• Nausea Fuel: The episode goes into horrifically grueling imagery and detail as Richard cuts off his legs and eats them, piece by piece.
