Werther had some sort of emotional disorder, which his love for Lotte only triggered
Most likely depression or some sort of anxiety. With psychology being in its infancy and psychiatry being flat-out nonexistent, it was basically dismissed as him just being melancholy/passionate, and by the time people finally notice that something was seriously wrong with Werther, it was too late.
