Everyone assumed that he died when he jumped off the cliff, but we know the siblings were bond to stick near their places of death, while he seems to have had no trouble continuing to lead the Trsanti in the years since his "death" and has apparently been spotted around the world. A character falling off a cliff when they Never Found the Body is usually a giant sign that they're really only Faking the Dead. In the version of the family portrait showing everyone in their true current forms he doesn't look at all monstrous or deformed, he just looks like the pirate that he is, and when you meet him in the flesh that remains true; it's only when he grabs the Gel'ziabar stone and starts overloading himself with its powers that he gains a monstrous form. That's plenty of evidence to suggest that he was perfectly alive and healthy until he had the most unbelievably bad timing ever in deciding to come back to plunder the family home (and that might even be because Jeremiah did something to lure him there at the right time for Patrick to take care of all ofhis siblings.)
