  • California Doubling: Toronto doubled for Los Angeles and Alaska.
  • Cast the Runner-Up: Shawn Roberts wanted the role of Chris Redfield, but it was given to Wentworth Miller. He then auditioned to replace Jason O'Mara as Albert Wesker, and this time he passed.
  • Fake American: The film has German-Ghanian Boris Kodjoe (Luther), English Kacey Clarke (Crystal), Spanish Sergio Peris-Mencheta (Ángel) and Canadians Shawn Roberts (Wesker) and Norman Yeung (Kim), all playing American characters.
  • Follow the Leader: Paul W.S. Anderson wanted to do the film in 3-D after watching Avatar.
  • Money, Dear Boy: According to Milla Jovovich, making a fourth film had been hotly debated; it was Extinction's economic success which settled it down.
  • No Stunt Double: Boris Kodjoe trained hard to do his own stunts as Luther. He still dislocated his knee doing one of them.
  • The Other Darrin: Shawn Roberts plays Albert Wesker instead of his previous actor, Jason O'Mara.
  • Role Reprise: Sienna Guillory reprises her role as Jill Valentine for the first time since 2004's Resident Evil: Apocalypse.
  • What Could Have Been:
    • Jensen Ackles was considered to play Leon S. Kennedy, but it didn't happen.
    • In the earliest versions of the scripts, there would be flashbacks showing the Redfield siblings's past together and their searches for each other. Alice would have also discovered Wesker's body among the debris after crashing the plane at the beginning, shooting it a few times to ensure he was dead, only for him to regenerate and show his Majini mouth after she was gone.
    • Paul W.S. Anderson wanted the film to be an optimistic ending, but Executive Meddling wanted a Sequel Hook that ensured more films, so he had to add the battle at the end and leave the ending open.
  • Word of God: The explanation for the Executioner and the Majini's presence? Wesker sent them.
