  • Blooper:
    • Elanos Haliat has anti-human views, which makes it otherwise odd when he shows up as a human in the UNC: Espionage Probe assignment. His name and a filter applied to his voice imply that he was meant to be a turian. This was confirmed when the Legendary Edition replaced his model with that of a turian.
    • If Captain Kirrahe dies on Virmire, but the rest of his squadron and Ashley survive, Ashley will still mention that "Kirrahe wants to brief [Shepard]" when talking to her about the mission.
  • Completely Different Title: The Bring Down the Sky DLC is known as Turbulences à 900 000 pieds (Turbulence at 900,000 feet) in French.
  • Cowboy BeBop at His Computer: The Cooper Lawrence and Fox News piece on the game, which falsely accused it of having interactive sex scenes and full digital nudity while also admitting they never played it. Lawrence retracted her comments a few months later after her latest book on Amazon was bombarded with 1 star reviews and after watching someone play the game.
  • Killer App: Microsoft handled publishing duties for the original Xbox 360 version, and it became a bestseller even before it came to PC.
  • Missing Episode: The source code for the Pinnacle Station DLC has been lost to history and the backups are unsalvageable, meaning it is not included in the PlayStation 3 port and the Legendary Edition.note 
  • Orphaned Reference: It was originally intended that Saren's appearance would evolve throughout the game; looking normal on Eden Prime, being augmented on Virmire, and looking like the corrupted cyborg we know him as in the endgame. This was ultimately cut due to development constraints, but not before this physical transformation was written into the script. As a result, the whole plot point of Shepard trying to prove that Saren has gone rogue comes off as a bit odd when neither them nor the Council pay any attention to the fact that Saren now looks Obviously Evil. Other things affected include Nihlus letting his guard down against Saren, which makes a bit more sense if Saren was supposed to look and sound just like his old self. Saren's big Motive Rant about him letting Sovereign fill his body with implants also works a bit better when you can actually see the effects of those implants progress.
  • What Could Have Been:
    • There's audio that reveals at some point, it was possible to save both Ashley and Kaidan while on Virmire.
    • There's dummied out audio that reveals it was possible to romance Kaidan with male Shepard to the point it's possible to hack it in and carry it forwards to later games. The third game finally gives you the option to do so normally.
    • Pinnacle Station was going to be included in the Legendary Edition, but the developers found out they didn't have the source code. After a lot of searching and contacting former team members, they were unable to salvage the backups.
