  • Creator Backlash: Holly Marie Combs on her House of Halliwell podcast referred to this as a terrible film, but admitted it was so bad it was good.
  • Creator-Chosen Casting: Larry Drake was always the first choice for the titular character, Manny Coto having loved his work as a killer Santa in a Tales from the Crypt episode.
  • Dawson Casting: While Holly Marie Combs's 19 doesn't really count, Glenn Quinn's 22 definitely does.
  • Fake American: Irish actor Glenn Quinn as Max.
  • Mid-Development Genre Shift: Manny Coto originally envisioned the film as a more serious story about a serial killer. The studio however were more interested in a traditional slasher, and so it became Denser and Wackier.
  • What Could Have Been:
    • The comic is essentially one big what could have been, since it was based off of earlier drafts of the script. Another early draft, titled Mr. Giggles, became the 2005 film Neighborhood Watch (also known as Deadly End), penned by the same writer. According to the writer, the only things kept for the final version were the character names and the surgery angle to the kills.
    • Lots of gore had to be cut to get an R-rating, since the MPAA were really cracking down on horror films in the early 90s.
    • While Larry Drake was the first choice for Dr Giggles, there was a point where Matt Frewer nearly got the part.
